【摘要】：目的探討峽部斷裂型腰椎滑脫的患者多裂肌MRI影像學(xué)改變,并與退變型腰椎滑脫及正常體檢健康者比較,發(fā)現(xiàn)其中規(guī)律,探討臨床價(jià)值。方法研究對(duì)象分為3組:峽部裂型組患者60例,其中L3滑脫3例、L4滑脫35例(Ⅰ度15例,Ⅱ度20例)、L5滑脫22例;退變型組患者60例,其中L3滑脫4例、L4滑脫32例(Ⅰ度20例,Ⅱ度12例)、L5滑脫24例;體檢資料健康者60例為對(duì)照組。所測(cè)對(duì)象的年齡及體重指數(shù)(BMI)進(jìn)行比較,并對(duì)所有患者行功能障礙指數(shù)評(píng)分(ODI)。應(yīng)用Image J軟件測(cè)量所納入對(duì)象的L3/4、L4/5、L5/S1椎間隙水平層面多裂肌凈橫截面面積占多裂肌區(qū)域橫截面面積(LCSA/GCSA)的比值。數(shù)據(jù)間的顯著性差異利用單因素方差分析(one-way ANOVA)或Student't檢驗(yàn)。結(jié)果峽部裂型組患者3個(gè)不同層面數(shù)據(jù)與退變型組相應(yīng)節(jié)段滑脫患者及對(duì)照組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),峽部裂型組L4不同程度滑脫患者3個(gè)不同層面數(shù)據(jù)與退變型組L4相應(yīng)程度滑脫患者比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論峽部裂型組與退變型組及對(duì)照組相比,其多裂肌退變明顯,可考慮其作為腰椎滑脫的診斷參考指標(biāo)。
