強(qiáng)直性脊柱炎全髖關(guān)節(jié)置換后隱性失血的危險(xiǎn)因素分析

【摘要】:背景:全髖關(guān)節(jié)置換是治療強(qiáng)直性脊柱炎累及髖關(guān)節(jié)破壞的有效措施,強(qiáng)直性脊柱炎患者全髖關(guān)節(jié)置換后有不同程度的貧血,而隱性失血在全髖關(guān)節(jié)置換圍手術(shù)期失血量占有較大比例,影響關(guān)節(jié)功能的恢復(fù)。目的:分析強(qiáng)直性脊柱炎髖關(guān)節(jié)受累患者接受全髖關(guān)節(jié)置換后隱性失血的危險(xiǎn)因素。方法:人工全髖關(guān)節(jié)置換治療的60例(70髖)強(qiáng)直性脊柱炎累及髖關(guān)節(jié)的患者,平均年齡35.12歲。根據(jù)Gross方程計(jì)算隱性失血量,收集和分析手術(shù)時(shí)間、血沉、C-反應(yīng)蛋白、體質(zhì)量指數(shù)、是否骨質(zhì)疏松、強(qiáng)直性脊柱炎髖關(guān)節(jié)放射指數(shù)、異體輸血量等對(duì)患者全髖關(guān)節(jié)置換后隱性失血的影響,并按隱性失血量分為高隱性失血組(≥480 m L)和低隱性失血組(480 mL),通過(guò)單因素相關(guān)分析及多因素Logistic回歸分析(SPSS17.0)影響強(qiáng)直性脊柱炎患者初次全髖關(guān)節(jié)置換術(shù)后高隱性失血的危險(xiǎn)因素。結(jié)果與結(jié)論:(1)患者初次全髖關(guān)節(jié)置換后隱性失血量為(737.76±419.18)mL,總失血量為(1 312.83±487.41)mL,隱性失血占總失血量的51.48%。高隱性失血組41髖,低隱性失血組29髖,比例為41∶29;(2)單因素相關(guān)分析結(jié)果顯示手術(shù)時(shí)間、強(qiáng)直性脊柱炎髖關(guān)節(jié)放射指數(shù)、是否骨質(zhì)疏松、異體輸血量、血紅蛋白下降量與高隱性失血顯著相關(guān);(3)多因素Logistic回歸分析結(jié)果顯示強(qiáng)直性脊柱炎髖關(guān)節(jié)放射指數(shù)、異體輸血量、血紅蛋白下降量與高隱性失血顯著相關(guān);(4)結(jié)果說(shuō)明,隱性失血量是強(qiáng)直性脊柱炎患者初次全髖關(guān)節(jié)置換后總失血量的重要組成部分,強(qiáng)直性脊柱炎髖關(guān)節(jié)放射指數(shù)、異體輸血量、血紅蛋白下降量是高隱性失血的危險(xiǎn)因素。
[Abstract]:Background: total hip replacement is an effective treatment for ankylosing spondylitis involving hip destruction. Patients with ankylosing spondylitis have different degrees of anemia after total hip replacement. Recessive blood loss accounts for a large proportion of total hip replacement perioperative blood loss, affecting the recovery of joint function. Objective: to analyze the risk factors of recessive blood loss after total hip replacement in patients with ankylosing spondylitis.
