TiRobot機(jī)器人輔助經(jīng)皮空心螺釘內(nèi)固定治療不穩(wěn)定型骨盆骨折

【摘要】:目的探討應(yīng)用骨科手術(shù)機(jī)器人輔助經(jīng)皮空心螺釘內(nèi)固定治療骨盆前后環(huán)損傷的手術(shù)方法及療效。方法利用第三代骨科手術(shù)機(jī)器人TiRobot系統(tǒng)于2016年11月至2017年2月對16例不穩(wěn)定骨盆骨折患者行機(jī)器人輔助經(jīng)皮空心螺釘內(nèi)固定骨盆前、后環(huán),觀察置釘位置,記錄手術(shù)時間、術(shù)中X線透視次數(shù)、切口長度、出血量、骨折愈合時間、術(shù)后功能等結(jié)果。結(jié)果平均手術(shù)時間65 min,平均透視次數(shù)14次。手術(shù)切口平均2 cm,平均術(shù)中出血量35 m L。所有置入螺釘位置精準(zhǔn)滿意,無一枚穿出骨皮質(zhì)。傷口均一期愈合,未出現(xiàn)醫(yī)源性血管、神經(jīng)及臟器損傷。術(shù)后隨訪3~6個月,平均4.2個月,無骨折延遲愈合或畸形愈合。末次隨訪Majeed評分平均為86.7分。結(jié)論 TiRobot系統(tǒng)輔助經(jīng)皮空心螺釘內(nèi)固定治療骨盆前后環(huán)損傷手術(shù)精確、安全,手術(shù)時間短,創(chuàng)傷小,對患者及醫(yī)務(wù)人員放射性損害小,臨床效果滿意,是微創(chuàng)治療不穩(wěn)定型骨盆骨折的理想方法。
[Abstract]:Objective to explore the surgical method and curative effect of orthopedic robot assisted percutaneous hollow screw fixation in the treatment of pelvic anterior and posterior ring injury. Methods from November 2016 to February 2017, 16 patients with unstable pelvic fractures were treated with the third generation orthopedic surgery robot TiRobot system. The anterior and posterior rings of pelvis were fixed with robotic assisted percutaneous hollow screws. The results of X-ray fluoroscopy, incision length, bleeding volume, fracture healing time, postoperative function and so on. Results the average operation time was 65 min and the average fluoroscopy frequency was 14 times. The average operative incision was 2 cm, and the average intraoperative bleeding was 35 mL. All screws were placed in a precise and satisfactory position, none of which pierced the bone cortex. All wounds healed at the first stage without iatrogenic vascular, nerve and organ injury. Follow-up for 3 ~ 6 months (mean 4.2 months) showed no delayed union or malunion. The average Majeed score at the last follow-up was 86.7. Conclusion TiRobot system assisted percutaneous hollow screw internal fixation for pelvic anterior and posterior ring injury is accurate, safe, short operation time, less trauma, less radiation damage to patients and medical personnel, and satisfactory clinical effect. It is an ideal method for the treatment of unstable pelvic fractures.
