【摘要】：目的利用Meta分析的方法綜合評(píng)價(jià)生物可吸收螺釘與普通皮質(zhì)骨螺釘在下脛腓聯(lián)合損傷的手術(shù)治療的療效評(píng)價(jià)。方法通過(guò)查找萬(wàn)方數(shù)據(jù)庫(kù)、維普數(shù)據(jù)庫(kù)、Pubmed及Embase等數(shù)據(jù)庫(kù),共納入12篇相關(guān)文獻(xiàn)。采集踝關(guān)節(jié)功能優(yōu)良率、踝關(guān)節(jié)功能評(píng)分、踝關(guān)節(jié)活動(dòng)度及相關(guān)并發(fā)癥等相關(guān)結(jié)果數(shù)據(jù),利用Rev Man5.35對(duì)結(jié)果進(jìn)行Meta分析。結(jié)果納入的12篇相關(guān)文獻(xiàn)均為隨機(jī)對(duì)照試驗(yàn),共872例患者,其中生物可吸收螺釘組(AS)447例及普通皮質(zhì)螺釘組(MS)425例。Meta分析結(jié)果顯示,與普通皮質(zhì)骨螺釘相比,生物可吸收螺釘固定術(shù)后踝關(guān)節(jié)功能優(yōu)良率更高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);踝關(guān)節(jié)功能評(píng)分兩組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);踝關(guān)節(jié)背屈及跖屈活動(dòng)度差異無(wú)統(tǒng)計(jì)學(xué)意義;生物可吸收螺釘固定術(shù)后并發(fā)癥的發(fā)生率更高,其中異物反應(yīng)發(fā)生率較普通皮質(zhì)骨螺釘組高,其余如斷釘、異位骨化、感染及創(chuàng)傷性關(guān)節(jié)炎等并發(fā)癥發(fā)生率與普通皮質(zhì)骨螺釘組差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論生物可吸收螺釘是治療下脛腓損傷的有效內(nèi)固定物,可有效避免普通皮質(zhì)骨螺釘二次手術(shù),同時(shí)也能減少斷釘?shù)娘L(fēng)險(xiǎn)及手術(shù)后感染發(fā)生的風(fēng)險(xiǎn),但同時(shí)也增加異物反應(yīng)發(fā)生率。
[Abstract]:Objective to evaluate the curative effect of bioabsorbable screw and common cortical screw in the treatment of inferior tibiofibular syndesmosis injury by Meta analysis. Methods by searching Wanfang database, Weip database, pubmed database and Embase database, 12 related literatures were included. The excellent and good rate of ankle function, ankle function score, ankle range of motion and related complications were collected, and the results were analyzed by Meta using Rev Man5.35. Results all the 12 articles involved were randomized controlled trials, including 447 cases of bioabsorbable screws group and 425 cases of normal cortical screw group. Meta-analysis showed that compared with normal cortical screw, 872 patients were compared with normal cortical screw. The excellent and good rate of ankle joint function after bio-absorbable screw fixation was higher, the difference was statistically significant (P 0.05), there was no significant difference in ankle function score between the two groups (P 0.05), and there was no significant difference in the motion of ankle dorsal flexion and metatarsal flexion. The incidence of complications after bioabsorbable screw fixation was higher than that in the common cortical screw group, and the others were broken nails and heterotopic ossification, the incidence of foreign body reaction was higher than that of the common cortical screw group. The incidence of complications such as infection and traumatic arthritis was not significantly different from that of normal cortical screw group. Conclusion the bioabsorbable screw is an effective internal fixator for the treatment of lower tibiofibular injury. It can effectively avoid the secondary operation of common cortical screw and reduce the risk of nail breakage and post-operative infection. But it also increased the incidence of foreign body reactions.
