【摘要】：目的研究大面積中重度燒傷患者給予切削痂植皮手術(shù)聯(lián)合納米銀敷料及光子治療儀輔助治療的療效。方法從2014年10月—2016年10月三峽大學(xué)仁和醫(yī)院燒傷整形科就診的大面積中重度燒傷患者中選取90例為研究對(duì)象,常規(guī)組45例行切削痂植皮手術(shù),研究組45例行切削痂植皮手術(shù)聯(lián)合納米銀敷料、光子治療儀輔助治療。比較兩組治療后創(chuàng)面愈合時(shí)間、發(fā)熱持續(xù)時(shí)間、血小板計(jì)數(shù)下降天數(shù)、治療效果、并發(fā)癥及死亡情況。結(jié)果治療后研究組創(chuàng)面愈合時(shí)間、發(fā)熱持續(xù)時(shí)間、血小板計(jì)數(shù)下降天數(shù)均比常規(guī)組短;研究組治療總有效率97.78%(44/45)高于常規(guī)組中的84.44%(38/45);研究組的創(chuàng)面侵襲性感染、膿毒癥、多器官功能障礙綜合征(MODS)、死亡率低于常規(guī)組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論切削痂植皮手術(shù)聯(lián)合納米銀敷料、光子治療儀輔助治療用于大面積中重度燒傷患者可縮短患者恢復(fù)時(shí)間,降低并發(fā)癥發(fā)生率。
[Abstract]:Objective to study the efficacy of escharectomy combined with nanometer silver dressing and photon therapy in large area moderate and severe burn patients. Methods from October 2014 to October 2016, 90 patients with moderate and severe burn were selected from Department of Burn and plastic surgery, Renhe Hospital, three Gorges University. 45 cases of routine group were treated with escharectomy and skin grafting. In the study group, 45 cases were treated with scab cutting and skin grafting combined with nano-silver dressing and photon therapy instrument. The wound healing time, fever duration, platelet count decreased days, therapeutic effect, complications and death were compared between the two groups. Results after treatment, the wound healing time, fever duration and platelet count in the study group were shorter than those in the routine group, the total effective rate of the study group was 97.78 / 44 / 45, which was higher than that in the conventional group (84.44 / 45), and the wound invasive infection and sepsis in the study group. The mortality of multiple organ dysfunction syndrome was lower than that of routine group (P 0.05). Conclusion cutting eschar skin grafting combined with nano-silver dressing and photon therapy can shorten the recovery time and reduce the incidence of complications in large area moderate and severe burn patients.
