【摘要】：目的觀察術(shù)后譫妄(postoperative delirium,POD)老年患者大腦灰質(zhì)體積與密度的異常。方法選擇擇期全麻下行非心臟手術(shù)的胸腹部手術(shù)老年患者47例,男26例,女21例,年齡60~75歲,ASAⅡ或Ⅲ級,于術(shù)后第3天行認知功能評估,根據(jù)是否發(fā)生POD分為POD組和C組,均行大腦磁共振成像(MRI)掃描。運用基于體素的形態(tài)測量學(voxel-based morphometry method,VBM)分析比較兩組灰質(zhì)體積和密度的差異,并將有差異腦區(qū)的相應(yīng)參數(shù)值與簡易精神狀態(tài)檢查(MMSE)評分行相關(guān)性分析。結(jié)果 POD組全腦灰質(zhì)體積明顯小于C組(P0.01),而腦脊液體積明顯大于C組(P0.01)。與C組比較,POD組雙側(cè)前額葉和右側(cè)海馬旁回的灰質(zhì)體積明顯減小(P0.01),雙側(cè)海馬和右側(cè)海馬旁回的灰質(zhì)密度明顯降低(P0.001),右側(cè)海馬旁回的灰質(zhì)體積與MMSE評分之間呈明顯正相關(guān)(P0.05)。結(jié)論額葉區(qū)域、海馬和海馬旁回的灰質(zhì)結(jié)構(gòu)異常可能在POD的發(fā)生發(fā)展過程中扮演著重要的角色,右側(cè)海馬旁回的灰質(zhì)體積可作為反映POD嚴重程度的參考指標之一。
[Abstract]:Objective to observe the abnormality of gray matter volume and density in aged patients with postoperative delirium POD. Methods Forty-seven elderly patients (26 males, 21 females, aged 60,75 years old) undergoing non-cardiac surgery under general anesthesia were enrolled in this study. The cognitive function was evaluated on the third day after operation. According to the occurrence of POD, they were divided into POD group and C group. Magnetic resonance imaging (MRI) was performed in all patients. The volume and density of gray matter in the two groups were analyzed and compared by voxel-based morphometry method based on voxel-based morphometry, and the correlation analysis was made between the corresponding parameters of the different brain area and the MMSE score of simple mental state examination. Results the volume of whole brain gray matter in POD group was significantly smaller than that in group C (P 0.01), while the volume of cerebrospinal fluid in group C was significantly larger than that in group C (P 0.01). Compared with group C, the gray matter volume of bilateral prefrontal lobe and right parahippocampal gyrus decreased significantly in group C, and the density of gray matter in bilateral and right hippocampal gyrus decreased significantly (P 0.001). There was a significant positive correlation between the volume of gray matter and MMSE score in right perihippocampal gyrus. Conclusion the abnormal gray matter structure in frontal lobe, hippocampus and perihippocampal gyrus may play an important role in the occurrence and development of POD. The volume of gray matter in right parahippocampal gyrus can be used as a reference index to reflect the severity of POD.
