【摘要】：目的探討應(yīng)用氣腹針穿線懸吊闌尾配合雙孔腹腔鏡闌尾切除術(shù)(LA)的安全性、可行性、技術(shù)要點(diǎn)及優(yōu)勢(shì)。方法回顧性分析應(yīng)用氣腹針穿線懸吊闌尾配合雙孔腹腔鏡闌尾切除術(shù)18例患者的臨床資料。結(jié)果 18例均手術(shù)成功,手術(shù)時(shí)間23~76 min,平均(32±6)min。術(shù)中出血量為4~10 m L,患者術(shù)后肛門排氣時(shí)間為6.5~20.5 h,平均(9.0±2.6)h。術(shù)后無(wú)腸瘺、切口感染、腹腔膿腫等近期并發(fā)癥發(fā)生。術(shù)后切口微創(chuàng)美觀。術(shù)后病理檢查示急性化膿性闌尾炎5例、急性單純性闌尾炎9例,慢性闌尾炎4例。全組患者獲得隨訪(門診隨訪7例、電話隨訪11例),術(shù)后平均隨訪時(shí)間為4個(gè)月(2~10個(gè)月),未發(fā)生戳孔疝、黏連性腸梗阻等遠(yuǎn)期并發(fā)癥,手術(shù)瘢痕不明顯。結(jié)論應(yīng)用氣腹針穿線懸吊闌尾配合雙孔腹腔鏡闌尾切除術(shù)是安全可行的,氣腹針穿線操作簡(jiǎn)單、經(jīng)濟(jì)實(shí)用,是一種簡(jiǎn)單易行的方法。
[Abstract]:Objective to explore the safety, feasibility, technical essentials and advantages of pneumoperitoneum puncture for suspending appendix and double hole laparoscopic appendectomy. Methods the clinical data of 18 cases of laparoscopic appendectomy with pneumoperitoneum thread suspension and double hole laparoscopic appendectomy were retrospectively analyzed. Results all the 18 cases were successfully operated, the operative time was 2376 minutes (mean 32 鹵6 minutes). The blood loss during operation was 4m / L, and the time of anal exhaust was 6.5 鹵20.5 h, with an average of 9.0 鹵2.6 hs. Postoperative intestinal fistula, incision infection, abdominal abscess and other recent complications occurred. The incision was minimally invasive and beautiful. Pathological examination showed acute suppurative appendicitis in 5 cases, acute simple appendicitis in 9 cases and chronic appendicitis in 4 cases. All patients were followed up (7 outpatients and 11 by telephone. The average follow-up time was 2 ~ 10 months after operation. There were no long term complications such as herniation of popliteal hernia, adhesive intestinal obstruction, etc.) the operation scar was not obvious. Conclusion it is safe and feasible to apply pneumoperitoneum needle in suspending appendicitis and double hole laparoscopic appendectomy. The operation of pneumoperitoneum needle puncture is simple, economical and practical, and it is a simple and feasible method.
