【摘要】：目的探討心臟驟停復(fù)蘇后患者體溫與腦功能預(yù)后的關(guān)系。方法本研究為多中心回顧性隊(duì)列研究,分析1990年1月1日~2011年12月31日北京市七家三甲醫(yī)院ICU收治的院內(nèi)心臟驟停后自主循環(huán)恢復(fù)超過(guò)20min的患者,收集復(fù)蘇治療相關(guān)數(shù)據(jù)和其他臨床資料,使用腦功能分級(jí)(CPC)評(píng)估腦功能結(jié)局,分析治療期間體溫與腦功能預(yù)后的關(guān)系。結(jié)果共納入184例患者,其中68例(37.0%)存活至出院,36例(19.6%)出院時(shí)腦功能良好(CPC≤2)。35.5℃入ICU后第一個(gè)24h內(nèi)最高體溫38.4℃是腦功能預(yù)后良好的獨(dú)立預(yù)測(cè)因素(OR=8.986,95%CI 1.156~69.882;P=0.036)。結(jié)論對(duì)于院內(nèi)心臟驟停后恢復(fù)自主循環(huán)的患者,入ICU后24h內(nèi)最高體溫維持在35.6~38.3℃有利于腦功能預(yù)后。
[Abstract]:Objective to investigate the relationship between body temperature and brain function prognosis after cardiac arrest and resuscitation. Methods from January 1, 1990 to December 31, 2011, a multicenter retrospective cohort study was conducted to analyze the patients who had recovered their autonomic circulation beyond 20min after cardiac arrest in seven third Class A Hospitals in Beijing from January 1, 1990 to December 31, 2011. The data of resuscitation therapy and other clinical data were collected to evaluate the outcome of brain function by using cerebral function grading (CPC) and to analyze the relationship between body temperature and prognosis of brain function during the treatment.
