【摘要】：目的探討逆向經(jīng)皮空心螺釘固定恥骨上支治療恥骨支內(nèi)、外側(cè)骨折的臨床效果。方法對(duì)21例存在恥骨上支不穩(wěn)定性骨折的患者進(jìn)行逆向經(jīng)皮空心螺釘固定,記錄手術(shù)及隨訪信息,并對(duì)恥骨上支內(nèi)、外側(cè)進(jìn)行對(duì)比,評(píng)估臨床效果。結(jié)果平均手術(shù)時(shí)間(67.7±15.2)min,其中恥骨上支內(nèi)側(cè)骨折為(55.3±14.4)min,恥骨上支外側(cè)骨折為(76.5±15.4)min,兩者差異有統(tǒng)計(jì)學(xué)意義(P0.05)。術(shù)中出血量平均(42.5±5.2)m L,內(nèi)、外側(cè)間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。術(shù)后平均隨訪(9.3±2.2)個(gè)月,所有患者均未出現(xiàn)周圍器官損傷、傷口感染、褥瘡、神經(jīng)損傷以及肌肉萎縮等并發(fā)癥。1例恥骨上支外側(cè)骨折病例出現(xiàn)內(nèi)固定周圍骨折,1例恥骨上支外側(cè)骨折病例出現(xiàn)退釘現(xiàn)象,其余所有患者骨折愈合良好。共16例患者完成Majeed評(píng)分,恥骨上支內(nèi)側(cè)骨折患者中5例優(yōu),2例良;外側(cè)骨折患者中5例優(yōu),3例良,1例可。結(jié)論逆向經(jīng)皮空心螺釘內(nèi)固定治療恥骨上支內(nèi)、外側(cè)骨折是一種安全、有效的治療方法。其中恥骨上支內(nèi)側(cè)骨折手術(shù)時(shí)間更短,但兩者臨床效果并無明顯差異。
[Abstract]:Objective to investigate the clinical effect of retrograde percutaneous hollow screw fixation of superior pubic branch in the treatment of medial and lateral pubic branch fractures. Methods 21 patients with unstable fracture of superior pubic branch were treated with reverse percutaneous hollow screw fixation, the information of operation and follow-up were recorded, and the medial and lateral superior pubic branch were compared to evaluate the clinical effect. Results the average operative time was 67.7 鹵15.2min, including 55.3 鹵14.4min in the medial fracture of the superior pubic branch and 76.5 鹵15.4 min in the lateral fracture of the suprapubic branch. The difference between the two groups was statistically significant (P 0.05). The average volume of intraoperative bleeding was 42.5 鹵5.2mL, and there was no significant difference between medial and lateral areas (P 0.05). The average follow-up period was 9.3 鹵2.2 months. No peripheral organ injury, wound infection, bedsore were found in all patients. Nerve injury and muscular atrophy. 1 case of lateral fracture of superior pubic branch appeared peripubic fracture and 1 case of lateral fracture of superior branch of pubis appeared denailing phenomenon. All the other patients had good fracture healing. Majeed score was completed in 16 patients, of whom 5 were excellent in medial fracture of superior pubic branch and 3 were good in lateral fracture. Conclusion Percutaneous internal fixation with hollow screw is a safe and effective method for the treatment of medial and lateral fractures of superior pubic branch. The operative time of medial superior pubic branch fracture was shorter, but there was no significant difference between them.
