【摘要】：目的研究CT血管造影(CTA)與彩色多普勒超聲成像技術(shù),對足趾供區(qū)血管情況進(jìn)行術(shù)前評估的臨床意義。材料和方法(1)術(shù)前采用彩色多普勒成像技術(shù),檢測25例手指缺損患者供區(qū)雙足背動脈、第一跖背動脈的走行、起始內(nèi)徑大小情況及與皮膚的距離。(2)25例手指缺損患者術(shù)前行雙足CT血管造影檢查,應(yīng)用計算機軟件初步建立三維數(shù)字化模型,將供區(qū)血管走行、毗鄰精準(zhǔn)定位,標(biāo)測、量化血管起始內(nèi)徑。結(jié)果25例患者足部第一跖背動脈均能在彩色多普勒和CTA檢測中顯現(xiàn)。術(shù)前兩種檢測方法所得血管走向、充盈情況與術(shù)中一致。第一跖背動脈起始內(nèi)徑平均值差異有統(tǒng)計學(xué)意義(P0.05),而皮下距離所得平均值差異無統(tǒng)計學(xué)意義(P0.05)。25例再造指均順利成活。結(jié)論CTA與彩色多普勒成像技術(shù)相結(jié)合用于足趾移植再造拇指術(shù)前檢查,能準(zhǔn)確反映供區(qū)血管的解剖學(xué)結(jié)構(gòu)特點并提供個體化的、清晰的足部血供圖像,對拇手指再造術(shù)術(shù)前評估足部供區(qū)血供具有重要的臨床價值。
[Abstract]:Objective to study the clinical significance of CT angiography (CTAA) and color Doppler ultrasound (CDFI) in preoperative evaluation of vascular status in the toe donor area. Materials and methods before operation, color Doppler imaging technique was used to detect the movement of the first dorsal metatarsal artery and the bilateral dorsal foot artery in the donor area of 25 patients with finger defect. The size of the initial inner diameter and the distance to the skin. 25 patients with finger defect were examined by bipedal CT angiography before operation. The three-dimensional digital model was established by computer software. The donor vessels were located and mapped accurately. The initial diameter of blood vessels was quantified. Results the first dorsal metatarsal artery could be detected by color Doppler and CTA in 25 patients. The blood vessel orientation obtained by the two methods before operation was consistent with that of intraoperative filling. There was significant difference in the initial diameter of the first dorsal metatarsal artery (P0.05), but there was no significant difference in the subcutaneous distance between the two groups. Conclusion CTA combined with color Doppler imaging can accurately reflect the anatomical features of donor vessels and provide individualized and clear images of blood supply to the foot for thumb reconstruction after toe transplantation. It is of important clinical value to evaluate the blood supply of foot region before thumb and finger reconstruction.
