【摘要】：目的研究分析顱腦手術(shù)治療患者術(shù)中自體血液回輸與異體輸血對(duì)其臨床療效及感染的影響。方法選取2014年1月-2016年1月醫(yī)院收治的行顱腦手術(shù)治療患者120例,根據(jù)術(shù)中輸血血液來(lái)源不同將患者分為自體輸血組和異體輸血組,各60例,觀察并記錄兩組患者術(shù)中出血量、自體血液回輸量、異體血液輸注量、血液輸入量;比較兩組患者術(shù)前、術(shù)后1、2、4、8d血紅蛋白、紅細(xì)胞計(jì)數(shù)、血小板計(jì)數(shù)等相關(guān)血液指標(biāo),術(shù)前、術(shù)后1、7d檢測(cè)炎性因子相關(guān)指標(biāo),術(shù)后觀察兩組感染情況。結(jié)果兩組患者術(shù)前相關(guān)檢測(cè)指標(biāo)均無(wú)顯著差異;自體輸血組術(shù)后1、4、8d紅細(xì)胞計(jì)數(shù)、血紅蛋白數(shù)值均顯著高于異體輸血組(P0.05);兩組術(shù)后1、4、8d血小板計(jì)數(shù)、凝血酶原時(shí)間、部分凝血活酶時(shí)間無(wú)顯著差異;兩組患者術(shù)前白介素-6、干擾素-γ檢測(cè)值無(wú)顯著差異;自體輸血組術(shù)后1、6d白介素-6和干擾素-γ檢測(cè)值顯著高于異體輸血組(P0.05);兩組患者輸血后不良反應(yīng)及感染情況比較,自體輸血組發(fā)生率(6.67%)顯著低于異體輸血組(16.67%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論顱腦手術(shù)術(shù)中行自體血液回收處理并回輸后對(duì)患者的免疫功能無(wú)抑制作用,紅細(xì)胞形態(tài)好,攜氧能力高,對(duì)術(shù)后感染的發(fā)生具有顯著的降低作用。
[Abstract]:Objective to study the effect of autologous blood transfusion and allogeneic blood transfusion on the clinical efficacy and infection in patients undergoing craniocerebral surgery. Methods 120 patients with craniocerebral surgery were selected from January 2014 to January 2016. According to the blood sources of blood transfusion during operation, the patients were divided into two groups: autotransfusion group and allogeneic transfusion group (60 cases each). The blood loss during operation was observed and recorded in the two groups. Autologous blood volume, allogeneic blood transfusion volume, blood input volume, hemoglobin, erythrocyte count and platelet count were compared between the two groups before and after operation. Postoperative infection was observed in both groups. Results there was no significant difference in preoperative correlation between the two groups, erythrocyte count and hemoglobin in autologous blood transfusion group were significantly higher than those in allogeneic blood transfusion group on the 8th day after operation, platelet count and prothrombin time at 1: 4 and 8 days after operation in the two groups were significantly higher than those in the allogeneic transfusion group. There was no significant difference in the time of partial thromboplastin between the two groups, and there was no significant difference between the two groups in the detection of interleukin-6 and interferon- 緯 before operation. The detection value of interleukin-6 and interferon-緯 in autologous blood transfusion group was significantly higher than that in allogeneic blood transfusion group on the 1st and 6th day after operation, and the incidence of adverse reaction and infection in autotransfusion group was significantly lower than that in allogeneic blood transfusion group (P 0.05), and the incidence of autotransfusion group was significantly lower than that in allogeneic transfusion group (P 0.05). Conclusion the recovery and transfusion of autologous blood during craniocerebral surgery have no inhibitory effect on the immune function of the patients, red blood cells have good morphology and high oxygen carrying capacity, and it can significantly reduce the incidence of postoperative infection.
