【摘要】：目的:探討復(fù)雜型腹主動(dòng)脈瘤(AAA)采用腔內(nèi)修復(fù)(EVAR)治療的操作要點(diǎn)和臨床療效的分析。方法:回顧性分析2011年1月至2016年6月安徽醫(yī)科大學(xué)第一附屬醫(yī)院血管外科收治的27例腹主動(dòng)脈瘤患者的臨床資料,分為復(fù)雜型腹主動(dòng)脈瘤組和普通型腹主動(dòng)脈瘤組,對(duì)兩組患者的術(shù)前一般資料、圍手術(shù)期治療及術(shù)后隨訪的進(jìn)行對(duì)比分析研究。結(jié)果:復(fù)雜組患者13例,普通組患者14例,復(fù)雜組平均年齡高于普通組患者的平均年齡(72.69±8.30)歲vs(70.21±8.54)歲,復(fù)雜組在疼痛癥狀例數(shù)、平均動(dòng)脈瘤體長(zhǎng)度、手術(shù)時(shí)長(zhǎng)、支架個(gè)數(shù)、住院費(fèi)用方面高于普通組(61、(9.10±2.10)cm(7.31±1.60)cm、(125±59)min(100±61)min、186310±33276162080±22412),兩組患者上訴資料的差異有統(tǒng)計(jì)學(xué)意義(p0.05);兩組患者手術(shù)成功率為100%,圍手術(shù)期內(nèi)漏數(shù)分別為5例和2例;術(shù)后兩組患者隨訪1~12個(gè)月,平均隨訪率為76.9%和71.4%,平均隨訪時(shí)間為(8.9±0.8)個(gè)月和(8.8±0.8)個(gè)月,隨訪期間發(fā)生內(nèi)漏的患者分別為2例和1例,期間未出現(xiàn)死亡病例,兩組患者的以上資料的差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)。結(jié)論:復(fù)雜型腹主動(dòng)脈瘤行腹主動(dòng)脈瘤腔內(nèi)修復(fù)術(shù)治療是一種切實(shí)可行的治療方法。普通型腹主動(dòng)脈瘤和復(fù)雜型腹主動(dòng)脈瘤兩組患者之間在疼痛癥狀例數(shù)、平均動(dòng)脈瘤體長(zhǎng)度、手術(shù)時(shí)長(zhǎng)、支架個(gè)數(shù)、住院費(fèi)用方面之間的差異有統(tǒng)計(jì)學(xué)意義。
[Abstract]:Objective: to investigate the operative points and clinical effect of intraluminal repair of complex abdominal aortic aneurysm (AAA). Methods: the clinical data of 27 patients with abdominal aortic aneurysm treated in the first affiliated Hospital of Anhui Medical University from January 2011 to June 2016 were retrospectively analyzed. The patients were divided into complex type abdominal aortic aneurysm group and common type abdominal aortic aneurysm group. The preoperative general data, perioperative treatment and postoperative follow-up were compared between the two groups. Results: there were 13 patients in complex group and 14 patients in common group. The average age of complex group was 72.69 鹵8.30 years vs(70.21 鹵8.54 years old than that of normal group. The number of pain symptoms, the length of artery tumor, the length of operation and the number of stents in complex group were higher than that in common group. The cost of hospitalization was higher than that of the normal group (9.10 鹵2.10)cm(7.31 鹵1.60m). The difference of appeal data between the two groups was statistically significant (p 0.05). The successful rate of operation was 100g / ml, the number of leakage in perioperative period was 5 cases and 2 cases were followed up for 1 ~ 12 months after operation, the patients in the two groups were followed up for 1 ~ 12 months, and the difference between the two groups was significant (p < 0.05). The average follow-up rate was 76.9% and 71.4%, and the average follow-up time was 8.9 鹵0.8 months and 8.8 鹵0.8 months, respectively. There were 2 cases of internal leakage and 1 case of internal leakage during the follow-up period. There was no significant difference in the above data between the two groups (p 0.05). Conclusion: endovascular repair of complex abdominal aortic aneurysm is a feasible method. There were significant differences in the number of pain symptoms, the average arterial tumor length, the length of operation, the number of stents and the cost of hospitalization between the patients with common abdominal aortic aneurysm and complex abdominal aortic aneurysm.
