【摘要】：[目的]探討應(yīng)用股骨近端防旋髓內(nèi)釘(proximal femoral nail antirotation,PFNA)內(nèi)固定治療股骨近端(股骨粗隆間、股骨粗隆下)骨折的臨床效果及治療體會(huì)。[方法]對(duì)90例在本院治療的股骨近端骨折患者,C臂機(jī)透視下行骨折閉合復(fù)位、PFNA內(nèi)固定微創(chuàng)治療。觀察骨折愈合時(shí)間、并發(fā)癥、髖關(guān)節(jié)功能恢復(fù)情況。[結(jié)果]手術(shù)時(shí)間20~90 min,平均40 min;術(shù)中出血量50~300 ml,平均80 ml。骨折全部愈合,無髖內(nèi)翻、螺釘退出、螺旋刀切出、感染、神經(jīng)血管損傷等并發(fā)癥發(fā)生。髖關(guān)節(jié)功能采用Harris評(píng)分,優(yōu)62例,良25例,可3例,優(yōu)良率達(dá)97%。[結(jié)論]PFNA內(nèi)固定微創(chuàng)治療股骨近端骨折操作簡單、損傷小、出血少、恢復(fù)快、并發(fā)癥少,是治療股骨近端骨折理想的治療方法。熟練掌握骨折復(fù)位技巧及PFNA內(nèi)固定手術(shù)技術(shù),可縮短手術(shù)時(shí)間,減少手術(shù)出血量,減少X線暴露次數(shù),對(duì)患者快速康復(fù)有重要意義。
[Abstract]:[objective] to investigate the clinical effect and treatment experience of proximal femoral nail antirotation nail in the treatment of proximal femur (intertrochanteric, subtrochanteric) fractures of the femur. [methods] A total of 90 patients with proximal femur fractures treated in our hospital were treated with closed reduction and minimally invasive treatment with PFNA internal fixation. The healing time, complications and hip function were observed. [results] the operative time was 20 ~ 90 min (mean 40 min) and the intraoperative bleeding was 50 ~ 300 ml (mean 80 ml). All fractures healed without hip varus, screw withdrawal, helical knife excision, infection, neurovascular injury and other complications. The hip joint function was assessed by Harris score, excellent in 62 cases, good in 25 cases, fair in 3 cases. The excellent and good rate was 97%. [conclusion] minimally invasive treatment of proximal femoral fractures with PFNA is an ideal method for the treatment of proximal femoral fractures with simple operation, less injury, less bleeding, quick recovery and less complications. Mastering the technique of fracture reduction and PFNA internal fixation can shorten the operation time, reduce the amount of operative bleeding and reduce the number of X-ray exposures, which is of great significance for the rapid recovery of patients.
