【摘要】：目的:研究不同椎間植骨材料治療脊髓型頸椎病和椎間盤突出癥的臨床療效分析。方法:回顧分析安徽醫(yī)科大學(xué)第一附屬醫(yī)院2011年9月至2013年8月頸椎間盤突出癥患者共63名,按照椎間植骨材料的不同將其分成同種異體骨環(huán)組(A組)男/女為12/11,Cage組(B組)男/女為11/7和自體髂骨組(C組)男/女為13/9,統(tǒng)計各樣本的年齡,性別,住院號,手術(shù)時間,出血量,并發(fā)癥,融合節(jié)段椎體前后高度差值(HAB/HPB),手術(shù)節(jié)段Cobb角和神經(jīng)功能改善(mJOA評分)。結(jié)果:1.所有患者均完成術(shù)后12-18個月的隨訪,三組患者術(shù)后6個月融合率分別為23.2%(A組),22.7%(B組),25.3%(C組),三組患者術(shù)后1年融合率分別為86.3%(A組),81.5%(B組),88.3%(C組)。2.A組與C組,B組與C組組間手術(shù)時間,術(shù)中出血量,HAB/HPB,Cobb角差異有統(tǒng)計學(xué)意義(P0.05),A組與B組組間手術(shù)時間,術(shù)中出血量,HAB/HPB,Cobb角無明顯差異(P0.05)。A組有1例出現(xiàn)骨環(huán)斷裂,B組有1例出現(xiàn)Cage移位,C組有4例出現(xiàn)髂骨區(qū)疼痛。結(jié)論:1.術(shù)后前3個月,自體髂骨的HAB/HPB差值和Cobb角改變最明顯,其次是同種異體骨,Cage變化最小。2.采用同種異體骨,Cage和自體髂骨作為椎間植骨材料治療頸脊髓壓迫的融合率相當(dāng),同種異體骨環(huán)和Cage可作為理想的椎間植骨材料。
[Abstract]:Objective: to study the clinical effect of different intervertebral bone graft materials in the treatment of cervical Spondylotic myelopathy and disc herniation. Methods: a total of 63 patients with cervical disc herniation from September 2011 to August 2013 in the first affiliated Hospital of Anhui Medical University were retrospectively analyzed. According to the different bone graft materials, the bone grafts were divided into two groups: group A (male / female: 12 / 11 Cage group, group B) (male / female = 11 / 7) and autogenous iliac bone group (group C) (13 / 9). The age, sex, hospitalization number and operation time of each sample were counted. Blood loss, complications, anterior and posterior height difference of fusion segment vertebrae, Cobb angle of surgical segment and improvement of nerve function were evaluated. The result is 1: 1. All the patients were followed-up 12-18 months after operation. The fusion rates of the three groups were 23.2and 22.7B, respectively. One year after operation, the fusion rates of the three groups were 86.3% and 81.5%, respectively, and the time of operation between group A and group C was higher than that between group B and group C, and the time of operation between group B and group C was higher than that of group C, respectively. There was significant difference in blood loss during operation between group A (P 0.05) and group B (P 0.05). There was no significant difference in blood loss during operation between group A and group B. there was no significant difference in blood loss during operation between group A and group B. There was no significant difference in the volume of blood loss between group A and group B (P 0.05). There was 1 case of bone ring rupture in group B and 1 case in group C had Cage shift. 4 cases had pain in iliac region. Conclusion 1. In the first 3 months after operation, the difference of HAB/HPB and Cobb angle of autogenous iliac bone was the most obvious, followed by the least change of Cage of allograft. The fusion rate of allograft Cage and autogenous iliac bone graft in the treatment of cervical spinal cord compression was similar, and allograft bone ring and Cage could be used as ideal intervertebral bone graft materials.
