【摘要】：目的探討腹腔鏡循肝靜脈入路解剖性肝切除的臨床療效。方法回顧性分析2013年1月至2016年12月的湖南省人民醫(yī)院微創(chuàng)外科97例腹腔鏡循肝靜脈入路解剖性肝切除術(shù)病例資料。結(jié)果 97例病人中,Pringle法阻斷70例,均為"15+5"模式,最多阻斷6次;鞘內(nèi)阻斷60例,鞘外阻斷24例。97例病人手術(shù)時間為(305.3±156.1)min,術(shù)中出血(133.3±170.4)mL,中轉(zhuǎn)開腹手術(shù)3例,術(shù)后胸腔積液4例,膽漏1例,傷口感染1例,均保守治療治愈,無再次手術(shù)和手術(shù)死亡病例。住院時間為(16.1±5.5)d。結(jié)論腹腔鏡循肝靜脈入路解剖性肝切除在器械、方法、技術(shù)保障下可以安全有效施行。
[Abstract]:Objective to investigate the clinical effect of anatomic hepatectomy by laparoscopic hepatic vein approach. Methods from January 2013 to December 2016, 97 cases of laparoscopic dissecting hepatectomy with hepatic vein approach in the minimally invasive surgery of Hunan Provincial people's Hospital were analyzed retrospectively. Results among 97 patients, 70 cases were blocked by Pringle method, all of them were "155" mode, the most was 6 times, intrathecal block was 60 cases, extrathecal occlusion was 24 cases, the operative time was 305.3 鹵156.1 min, intraoperative bleeding was 133.3 鹵170.4 mL, conversion to open operation was 3 cases, postoperative pleural effusion was 4 cases, operation time was 305.3 鹵156.1 min, intraoperative hemorrhage was 133.3 鹵170.4 mL, operation was converted to open operation in 3 cases, and postoperative pleural effusion in 4 cases. There were 1 case of bile leakage and 1 case of wound infection. The length of hospitalization was 16.1 鹵5.5 days. Conclusion Laparoscopic transhepatic hepatic resection via hepatic vein approach can be performed safely and effectively with instruments, methods and techniques.
