【摘要】：目的:探討髖關(guān)節(jié)腔內(nèi)注射氨甲環(huán)酸結(jié)合夾閉引流管對減少全髖關(guān)節(jié)置換術(shù)失血的有效性和安全性。方法:回顧性研究山東中醫(yī)藥大學(xué)附屬醫(yī)院2015年3月至2016年12月期間符合要求的單側(cè)全髖關(guān)節(jié)置換術(shù)的患者120例,其中試驗組60例,局部應(yīng)用氨甲環(huán)酸;對照組60例,不應(yīng)用氨甲環(huán)酸。兩組均在關(guān)節(jié)腔內(nèi)放置引流管一根并采用自體血回輸器。對兩組患者術(shù)后引流量、失血量、輸血量、Harris髖關(guān)節(jié)評分及相關(guān)并發(fā)癥等,進行統(tǒng)計學(xué)分析。結(jié)果:局部應(yīng)用氨甲環(huán)酸組與對照組比較,在性別、年齡、體重指數(shù)等一般資料方面無明顯差異(P0.05)。局部應(yīng)用氨甲環(huán)酸組比對照組在術(shù)后引流量、術(shù)后3d的顯性失血量、總失血量、輸血量等方面均有所減少,差異具有統(tǒng)計學(xué)意義(P0.05),在隱性失血量方面無統(tǒng)計學(xué)差異(P0.05)。在術(shù)后并發(fā)癥方面,兩組患者在DVT、下肢淋巴水腫、皮下瘀斑、肺部感染等方面比較,其差異均無統(tǒng)計學(xué)意義(P0.05)。在術(shù)后3月髖關(guān)節(jié)Harris評分方面,兩組無顯著差異(P0.05),且評分均有所提高。結(jié)論:在全髖關(guān)節(jié)置換術(shù)中,關(guān)節(jié)腔內(nèi)注射氨甲環(huán)酸結(jié)合短期夾閉流管能夠減少術(shù)后引流量、總失血量、異體血使用量,且不增加并發(fā)癥的發(fā)生率,不影響術(shù)后髖關(guān)節(jié)功能的恢復(fù)。
[Abstract]:Objective: to investigate the efficacy and safety of intra-articular injection of aminocyclylic acid combined with clipping drainage tube in reducing blood loss in total hip arthroplasty (THR). Methods: a retrospective study of 120 patients with unilateral total hip replacement (THR) from March 2015 to December 2016 in the affiliated Hospital of Shandong University of traditional Chinese Medicine, including 60 cases in the experimental group, 60 cases in the experimental group and 60 cases in the control group, No carbamate is used. One drainage tube was placed in the articular cavity and autologous blood transfusion device was used in both groups. The postoperative drainage, blood loss, Harris hip score and related complications were analyzed statistically. Results: there was no significant difference in sex, age and body mass index between the two groups. The amount of dominant blood loss, the total amount of blood loss and the amount of blood transfusion in the local application group were lower than those in the control group. The difference was statistically significant (P 0.05), but there was no significant difference in the hidden blood loss (P 0.05). In terms of postoperative complications, there was no significant difference in DVT, lower extremity lymphedema, subcutaneous ecchymosis and pulmonary infection between the two groups. There was no significant difference in Harris score of hip joint between the two groups at 3 months after operation (P 0.05), and the score was improved. Conclusion: in total hip arthroplasty, intraarticular injection of carbamoic acid combined with short-term clipping can reduce postoperative drainage, total blood loss, allogeneic blood consumption, and do not increase the incidence of complications. The recovery of hip function was not affected after operation.
