【摘要】：目的:探討跨傷椎經(jīng)皮與開放內(nèi)固定治療Magerl A型胸腰椎骨折的療效。方法:回顧性分析56例無神經(jīng)癥狀的單節(jié)段Magerl A型胸腰椎骨折患者,微創(chuàng)組26例,開放組30例。對兩組手術(shù)時間、術(shù)中出血、切口長度、術(shù)后下地時間、Cobb角、傷椎前緣高度比值、VAS評分、ODI評分等指標(biāo)進行比較分析。結(jié)果:微創(chuàng)組術(shù)中出血量、切口長度、術(shù)后下地時間均低于開放組(P0.05),術(shù)后3 d及末次隨訪時VAS評分及ODI評分均低于開放組(P0.05),兩組術(shù)后3 d和末次隨訪時傷椎前緣高度比值、Cobb角、VAS評分和ODI評分均較術(shù)前改善(P0.05),末次隨訪時的傷椎前緣高度比值、Cobb角均較術(shù)后3 d丟失(P0.05)。結(jié)論:治療無神經(jīng)癥狀的Magerl A型胸腰椎骨折微創(chuàng)手術(shù)可取得與開放相似的效果,且具有創(chuàng)傷小、出血少、下地早、恢復(fù)快等優(yōu)點。
[Abstract]:Objective: to evaluate the effect of percutaneous and open internal fixation on Magerl type A thoracolumbar fractures. Methods: 56 patients with single segment Magerl A thoracolumbar fracture without neurological symptoms were retrospectively analyzed, 26 cases in the minimally invasive group and 30 cases in the open group. The operative time, intraoperative bleeding, incision length, postoperative time of landing and Cobb angle, VAS score and ODI score were compared between the two groups. Results: in the minimally invasive group, the amount of intraoperative bleeding, the length of incision, The time after operation was lower than that in the open group (P 0.05), and the VAS score and the ODI score at the 3rd day and the last follow-up were lower than those in the open group. The ratio of anterior height of the injured vertebrae on the 3rd day and the last follow-up in the two groups was higher than that in the preoperative group. At the last follow-up, the ratio of anterior height of injured vertebrae and Cobb angle were all higher than that of 3 days after operation. Conclusion: minimally invasive treatment of Magerl A thoracolumbar fractures without neurologic symptoms can achieve similar results with open operation, and has the advantages of less trauma, less bleeding, early descent and quick recovery.
