【摘要】：目的：探討應(yīng)用鎖定鋼板與微創(chuàng)內(nèi)固定系統(tǒng)(LISS)治療股骨遠(yuǎn)端骨折的并發(fā)癥和早期臨床效果療效差異。方法：收集了2012年9月-2014年8月新疆醫(yī)科大學(xué)第五附屬醫(yī)院采用鎖定鋼板與LISS鋼板治療的82例股骨遠(yuǎn)端骨折患者的臨床資料。根據(jù)納入標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn)篩選患者60人,其中鎖定鋼板者24例,LISS板36例,年齡20-72歲,從手術(shù)時間、出血量、手術(shù)切口長度、住院時間以及骨折的愈合時間、膝關(guān)節(jié)評分Evanich評分五個方面的評價對2種治療方法進(jìn)行比較分析。結(jié)果:LISS與鎖定鋼板組相比,LISS組切口長度短,術(shù)中出血量少,手術(shù)時間短(P0.05),2組骨折愈合時間、住院時間比較有統(tǒng)計學(xué)意義。術(shù)后4周發(fā)生感染1例(鎖定鋼板組),骨折不愈合1例(LISS鋼板組),因患膝關(guān)節(jié)活動度降低行膝關(guān)節(jié)鏡下松解術(shù)1例(鎖定鋼板組)。LISS鋼板組Evanich評分總分為(90.6±3.3)分,高于鎖定鋼板組的(81.2±1.8)分。結(jié)論：微創(chuàng)剛板是治療股骨遠(yuǎn)端骨折的理想內(nèi)固定方式。
[Abstract]:Objective: to investigate the complications and early clinical effects of locking plate and minimally invasive internal fixation system (LISS) in the treatment of distal femoral fractures. Methods: the clinical data of 82 patients with distal femoral fractures treated with locking plate and LISS plate in the Fifth affiliated Hospital of Xinjiang Medical University from September 2012 to August 2014 were collected. Sixty patients were selected according to the inclusion criteria and exclusion criteria, including 24 cases of locking plate with Liss plate, aged 20-72 years, from operative time, bleeding volume, length of incision, length of hospital stay and healing time of fracture. The evaluation of knee joint score (Evanich) in five aspects was used to compare and analyze the two treatment methods. Results compared with the locking plate group, the length of incision was shorter, the amount of intraoperative bleeding was less, the operative time was shorter than that of the locking plate group, and the time of fracture healing and hospitalization were significantly different between the two groups. Infection occurred in 1 case 4 weeks after operation (locking plate group, fracture nonunion group 1 case, Liss plate group 1 case), knee arthroscopic lysis was performed in 1 case (the total score of Evanich score of locking plate group was 90.6 鹵3.3), and that of Liss plate group was 90.6 鹵3.3%, the total score of Liss plate group was 90.6 鹵3.3%, and the total score of Liss plate group was 90.6 鹵3.3%. It was higher than that of locking plate group (81.2 鹵1.8). Conclusion: minimally invasive rigid plate is an ideal internal fixation for distal femoral fracture.
