【摘要】：目的比較內(nèi)外固定結(jié)合(有限內(nèi)固定結(jié)合外固定支架)與雙側(cè)鋼板內(nèi)固定在治療復雜性脛骨平臺骨折治療中的療效及安全性。方法選取2013年8月-2016年8月于該院接受復雜脛骨平臺骨折手術(shù)的患者共68例,將其隨機分為兩組。其中觀察組36例,采用內(nèi)外固定結(jié)合手術(shù)治療,對照組32例,采用雙側(cè)鋼板內(nèi)固定手術(shù)。觀察術(shù)中情況、術(shù)后并發(fā)癥、骨折愈合時間及膝關(guān)節(jié)功能評分等。結(jié)果兩組患者在手術(shù)時間、術(shù)中出血量、骨折愈合時間及膝關(guān)節(jié)功能評分等方面比較,差異有統(tǒng)計學差異(P0.05),觀察組優(yōu)于對照組。術(shù)后并發(fā)癥方面,兩組僅切口感染比較,差異有統(tǒng)計學意義(P0.05),觀察組優(yōu)于對照組,而延遲愈合和創(chuàng)傷性關(guān)節(jié)炎兩組比較,差異無統(tǒng)計學意義(P0.05)。結(jié)論與雙側(cè)鋼板內(nèi)固定相比,內(nèi)外固定結(jié)在治療復雜性脛骨平臺骨折中安全有效,且并發(fā)癥較少。
[Abstract]:Objective to compare the efficacy and safety of internal and external fixation (limited internal fixation combined with external fixation) and bilateral plate fixation in the treatment of complex tibial plateau fractures. Methods Sixty-eight patients with complicated tibial plateau fractures were randomly divided into two groups from August 2013 to August 2016. 36 cases in the observation group were treated with internal and external fixation combined with surgery, while 32 cases in the control group were treated with bilateral plate internal fixation. The intraoperative condition, postoperative complications, fracture healing time and knee function score were observed. Results there were significant differences in operation time, intraoperative bleeding volume, fracture healing time and knee joint function score between the two groups. The observation group was better than the control group. In terms of postoperative complications, there was a significant difference between the two groups only in incision infection, the observation group was superior to the control group, while the delayed healing and traumatic arthritis groups had no significant difference (P 0.05). Conclusion compared with bilateral plate internal fixation, internal and external fixation is safe and effective in the treatment of complex tibial plateau fractures, with fewer complications.
