【摘要】：[目的]觀察復(fù)合有富血小板血漿(platlet rich plasma,PRP)明膠海綿復(fù)合物的自體肌腱移植物對(duì)前交叉韌帶重建術(shù)后骨隧道與肌腱間愈合過程的影響,探討其在腱與骨愈合中發(fā)揮的效果。[方法]新西蘭大白兔36只,無(wú)菌條件下獲取自體PRP,制備PRP明膠海綿復(fù)合物,左下肢行自體跟腱移植前交叉韌帶重建術(shù)。A組(實(shí)驗(yàn)組)使用PRP明膠海綿復(fù)合物;B組(對(duì)照組1)使用PRP;C組(對(duì)照組2)僅為肌腱。植入2、4、8、12周后,制作標(biāo)本,進(jìn)行大體及HE、Masson染色和TGF-β1免疫組化觀察。[結(jié)果]A組標(biāo)本大體觀察:前交叉韌帶連續(xù)性及張力良好,術(shù)后2周骨隧道口見少量瘢痕組織;4周骨道口見小部分骨樣組織,8周骨道口較多瘢痕呈現(xiàn),12周骨道口現(xiàn)較多骨樣成分。組織學(xué)觀察:HE染色顯示,A組出現(xiàn)Sharpey樣纖維和軟骨細(xì)胞明顯早于B組和C組;同樣在Masson結(jié)果中,A組出現(xiàn)有規(guī)則的膠原纖維也明顯多于B組和C組。TGF-β1免疫組織化學(xué)觀察:2、4、8周,A組陽(yáng)性率明顯高于B、C組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。而在術(shù)后12周時(shí),各組之間的表達(dá)無(wú)明顯差異(P0.05)。[結(jié)論]PRP明膠海綿復(fù)合物可促進(jìn)前交叉韌帶重建后腱骨的早期愈合。
[Abstract]:[objective] to observe the effect of autologous tendon grafts combined with platlet rich plasma-PRP sponge complex on the healing process between bone tunnel and tendon after anterior cruciate ligament reconstruction, and to explore its effect on tendon and bone union.[methods] PRP gelfoam complex was prepared from 36 New Zealand white rabbits.Anterior cruciate ligament reconstruction with autologous Achilles tendon transplantation in left lower extremity. Group A (experimental group) was treated with PRP gelfoam complex B (control group 1) and group C (control group 2) was only tendon.After 12 weeks of implantation, the specimens were made, and the gross and HE Masson staining and TGF- 尾 1 immunohistochemistry were observed.[results] in group A, there was good continuity and tension of anterior cruciate ligament.At 2 weeks after operation, a small amount of scar tissue was found at the entrance of bone tunnel. At 4 weeks after operation, a small part of bone tissue was found in the orifice of bone. More scars appeared in the orifice of bone at 8 weeks after operation, and there were more bone like components in the orifice of bone at 12 weeks after operation.Histological observation showed that Sharpey like fibers and chondrocytes appeared earlier in group A than those in group B and group C.In the same Masson results, the number of regular collagen fibers in group A was significantly higher than that in group B and group C, and the positive rate in group A was significantly higher than that in group B and group C (P 0.05), and the positive rate in group A was significantly higher than that in group B (P 0.05).At 12 weeks after operation, there was no significant difference in the expression of P0. 05.[conclusion] PRP gelfoam complex can promote the early healing of tendon bone after anterior cruciate ligament reconstruction.
