右美托咪定對淤膽并肝纖維化大鼠肝臟缺血再灌注損傷的影響

【摘要】:目的:探討右美托咪定(Dexmedetomidine,Dex)對膽汁淤積并肝纖維化大鼠肝臟缺血再灌注損傷(Ischemia Reperfusion Injury,IRI)的影響。方法:采用膽總管結(jié)扎(Bile Duct Ligation,BDL)法建立淤膽并肝纖維化大鼠模型,將32只雄性成模SD大鼠隨機分為4組,每組8只:①假手術(shù)組(S組):開腹顯露肝蒂,經(jīng)腹腔注射生理鹽水(Nromal Saline,NS)5mL/kg,不阻斷肝門,60min后關(guān)腹,術(shù)畢4h后再次開腹,取血液及肝組織標(biāo)本并處死大鼠。②缺血再灌注組(IRI組):開腹顯露肝蒂,經(jīng)腹腔注射NS 5mL/kg,30min后阻斷肝門,造成肝臟缺血,30min后解除阻斷并關(guān)腹,后續(xù)操作同S組。③10μg/kg Dex+缺血再灌注組(D10組):開腹顯露肝蒂,經(jīng)腹腔注射2μg/mL濃度Dex 5mL/kg,30min后阻斷肝門,后續(xù)操作同IRI組。④100μg/kg Dex+缺血再灌注組(D100組):開腹顯露肝蒂,經(jīng)腹腔注射20μg/mL濃度Dex 5mL/kg,30min后阻斷肝門,后續(xù)操作同IRI組。檢測指標(biāo):取血清檢測總膽紅素(Total Billirubin,TBIL)、直接膽紅素(Direct Billirubin,DBIL)、谷草轉(zhuǎn)氨酶(Aspertate Transaminase,AST)、谷丙轉(zhuǎn)氨酶(Alanine Transaminase,ALT)及腫瘤壞死因子-α(Tumor Necrosis Factor-α,TNF-α)濃度,取新鮮肝組織測定超氧化物歧化酶(Superoxide Dismutase,SOD)、丙二醛(Malondialdehyde,MDA)濃度;制作肝組織石蠟切片,行HE染色及Masson染色觀察肝組織病理形態(tài)學(xué)改變。結(jié)果:①血清TBIL及DBIL水平:4組TBIL結(jié)果間差異及DBIL結(jié)果間差異均無統(tǒng)計學(xué)意義(P0.05)。②血清AST及ALT濃度:IRI組、D10組及D100組AST均明顯高于S組(P0.05),D10組及D100組AST較IRI組明顯下降(P0.05),且D100組AST明顯低于D10組(P0.05);IRI組ALT明顯高于S組(P0.05),D100組ALT明顯低于IRI組(P0.05)。③肝組織SOD活性及MDA濃度:IRI組、D10組及D100組SOD均明顯低于S組(P0.05),D10組及D100組SOD均明顯高于IRI組(P0.05);IRI組、D10組及D100組MDA均明顯高于S組(P0.05),D10組及D100組MDA均明顯低于IRI組(P0.05)。④血清TNF-α濃度:IRI組、D10組及D100組TNF-α均明顯高于S組(P0.05),D10組及D100組TNF-α均明顯低于IRI組(P0.05)。⑤肝組織病理改變:4組肝組織病理切片均出現(xiàn)相同程度膽管增生及纖維化改變,IRI組肝竇內(nèi)可見明顯炎癥細胞浸潤,肝細胞腫脹,甚至出現(xiàn)壞死灶,而D10組及D100組肝組織損傷程度較IRI組輕。結(jié)論:右美托咪定可能通過上調(diào)SOD活性、下調(diào)TNF-α濃度減輕淤膽并肝纖維化大鼠肝臟缺血再灌注損傷。
[Abstract]:Aim: to investigate the effect of Dexmedetomine Dexine on hepatic ischemia reperfusion injury in rats with cholestasis and hepatic fibrosis.Methods: the rat model of cholestasis and hepatic fibrosis was established by bile duct ligation and bile duct ligation. Thirty-two male SD rats were randomly divided into 4 groups.After 30 minutes of liver ischemia, the liver was unblocked and closed. The subsequent operation was the same as group S (.310 渭 g/kg Dex ischemia / reperfusion group): the liver pedicle was exposed by laparotomy, and the hepatic hilus was blocked by intraperitoneal injection of 2 渭 g/mL Dex (5 mL / kg) for 30 minutes.The follow-up operation was similar to that in the IRI group (.4100 渭 g/kg Dex ischemia / reperfusion group): the liver pedicle was exposed by laparotomy, and the hepatic hilus was blocked by intraperitoneal injection of 20 渭 g/mL Dex 5 mL / kg for 30 minutes. The follow-up operation was the same as that in the IRI group.The pathological changes of liver tissue were observed by HE staining and Masson staining.Results there was no significant difference in serum TBIL and DBIL levels between the four groups of TBIL and DBIL results. There was no significant difference in serum AST and ALT concentration. The AST of D10 group and D100 group was significantly higher than that of S group P0.05% D10 group and D100 group AST was significantly lower than that of IRI group.The pathological sections of liver tissues in the 4 groups showed the same degree of bile duct hyperplasia and fibrosis. The infiltration of inflammatory cells in the hepatic sinuses of IRI group was obvious.Liver cells were swollen and even necrotic, but the degree of liver tissue injury in group D 10 and D 100 was less than that in group IRI.Conclusion: dexmetomidine may reduce hepatic ischemia-reperfusion injury in rats with cholestasis and hepatic fibrosis by upregulating the activity of SOD and down-regulating the concentration of TNF- 偽.
