【摘要】：目的:探討腰椎間盤退變的位置、范圍、程度、與脊柱骨盆矢狀位參數(shù)的關系。方法:回顧性分析2013年1月至2015年12月間76例脊柱外科腰椎間盤退變疾病(腰椎間盤突出癥、退變性腰椎不穩(wěn)癥)患者臨床資料。統(tǒng)計:1、患者一般資料:姓名、性別、年齡、身高、體重;2、脊柱全長片上測量矢狀位參數(shù)(TK、LL、SS、PT、PI、SVA)3、腰椎間盤退變的位置、范圍、程度。對各參數(shù)進行統(tǒng)計分析,按性別、PI的大小進行分組分析,同時腰椎間盤退變的位置、范圍、程度與脊柱骨盆矢狀位參數(shù)行相關性分析。結果:(1)男性患者和女性患者平均年齡分別是58.86歲、53.28歲(P㩳0.05),退變的位置值、范圍值、程度值分別是3.74±0.78、3.92±1.07(P㧐0.05)、2.41±1.36、1.81±1.01(P㩳0.05)、3.73±0.73、3.38±0.54(P㩳0.05)。(2)年齡與TK(r=0.376,P㩳0.01)、年齡與退變位置(r=-0.358,P㩳0.01)、年齡與退變范圍(r=0.329,P㩳0.01)、年齡與退變程度(r=0.452,P㩳0.01);TK與LL(r=0.531,P㩳0.01);LL和PI(r=0.409,P㩳0.01);LL與SS(r=0.827,P㩳0.01)、PI與SS(r=0.532,P㩳0.01)、PI與PT(r=0.711,P㩳0.01);PI與退變性位置(r=-0.257,P㩳0.05);SVA與退變的范圍(r=0.304,P㩳0.01)、SVA與退變程度(r=0.353,P㩳0.01)。(3)按PI大小分組后,PI≤50°組患者位置值為4.14±0.64,PI50°組患者位置值為3.57±1.08(P㩳0.05)。結論:1、隨退變的發(fā)生,脊柱骨盆矢狀面不斷代償來維持站立時的重心,減少能量消耗。2、腰椎間盤退變的位置、范圍、程度與年齡密切相關。3、腰椎間盤退變的位置多發(fā)生于下腰椎,更具體位置受脊柱骨盆矢狀面形態(tài)的影響,小PI人群傾向于L4/5、L5/S1椎間盤的退變,大PI人群更傾向于L3/4,L4/5椎間盤的退變。
