【摘要】：目的探討腰椎體終板下骨髓脂肪沉積與椎間盤退變Pfirrmann分級(jí)的相關(guān)性。方法采用橫斷面隊(duì)列研究方法,納入研究105名志愿者,共525個(gè)椎間盤(L_(1/2)~L_5/S_1),按不同年齡段分三組,青年組(20~39歲),中年組(40~54歲),老年組(55~70歲)。對(duì)每個(gè)椎間盤行Pfirrmann分級(jí),并采用改良水-脂分離技術(shù)(m DixonQuant)方法定量測(cè)量每個(gè)椎間盤相應(yīng)上位、下位椎體終板下的脂肪分?jǐn)?shù)(FF)值;對(duì)椎體骨髓FF值及椎間盤Pfirrmann分級(jí)進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果不同腰椎間盤Pfirrmann分級(jí)間其上位、下位椎體終板下骨髓FF值均有顯著性差異(上位椎體F=78.806,P=0.000,下位椎體F=75.459,P=0.000)。控制年齡因素,上位、下位椎體骨髓FF與腰椎間盤Pfirrmann分級(jí)均沒(méi)有相關(guān)性(上位椎體:r值為0.131,P0.05;下位椎體:r值為0.111,P0.05)。不同年齡組上位椎體及下位椎體骨髓FF值與Pfirrmann分級(jí)相關(guān)性不確切;不同年齡組間及組內(nèi)腰椎間盤上位及下位椎體骨髓FF值均有顯著性差異(上位椎體F=265.090,P=0.000,下位椎體F=305.890,P=0.000)。結(jié)論目前尚無(wú)證據(jù)證明上位及下位椎體終板下骨髓FF值與其對(duì)應(yīng)節(jié)段椎間盤退變Pfirrmann分級(jí)之間存在相關(guān)性,定量測(cè)量退變椎間盤上位及下位椎體終板下骨髓脂肪含量尚不能對(duì)椎間盤退變程度分級(jí)進(jìn)行有效補(bǔ)充。
[Abstract]:Objective to investigate the correlation between bone marrow fat deposition and Pfirrmann grading of lumbar intervertebral disc degeneration.Methods A cross-sectional cohort study was conducted in 105 volunteers. A total of 525 patients were divided into three groups: young group (2039 years old), middle-aged group (4054 years old) and elderly group (5570 years old).Pfirrmann classification was performed on each disc, and the fat fraction of each disc was measured quantitatively by using the modified water-lipid separation technique.The FF value of vertebral bone marrow and Pfirrmann grading of intervertebral disc were analyzed statistically.Results there were significant differences in FF value of bone marrow between different Pfirrmann classification of lumbar intervertebral disc and inferior vertebral body endplate (P < 0. 000 in upper vertebral body and 75. 459% in lower vertebral body respectively), in upper vertebral body (78.806) and in lower vertebra (75. 459%), there was no significant difference in FF value between upper and lower lumbar intervertebral disc (P < 0. 000).There was no correlation between bone marrow FF and Pfirrmann grading of lumbar intervertebral disc in upper and lower vertebrae (P 0.05 for upper vertebral body and 0.131% for upper vertebra, 0.111% for lower vertebral body and 0.111% for lower vertebra).Conclusion at present, there is no evidence to prove the correlation between the FF value of bone marrow in superior and inferior vertebral body and the corresponding Pfirrmann grading of intervertebral disc degeneration.Quantitative measurement of bone marrow fat content in superior and inferior endplate of degenerative intervertebral disc can not effectively supplement the grading of degenerative degree of intervertebral disc.
