【摘要】：目的:評價(jià)小腦裂入路在三叉神經(jīng)微血管減壓術(shù)中的應(yīng)用意義,從中獲取更多相關(guān)證據(jù)及經(jīng)驗(yàn),指導(dǎo)臨床應(yīng)用。方法:選取神經(jīng)外科2012年1月-2013年12月期間行微血管減壓術(shù)的原發(fā)性三叉神經(jīng)痛病例167例。根據(jù)納入排除標(biāo)準(zhǔn),共收集到傳統(tǒng)枕下乙狀竇后入路組(A組)病例53例和小腦裂入路組(B組)病例61例,對比分析兩種術(shù)式在治療原發(fā)性三叉神經(jīng)痛中三叉神經(jīng)根暴露情況、巖靜脈阻擋情況、巖靜脈燙除情況、責(zé)任血管的壓迫部位、責(zé)任血管類型、術(shù)后并發(fā)癥及術(shù)后第1天、第12個(gè)月、第24個(gè)月、第36個(gè)月的療效,并進(jìn)行統(tǒng)計(jì)分析。結(jié)果:A、B組的一般臨床資料、疼痛支分布情況差異無統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)中三叉神經(jīng)根暴露情況差異有統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)中巖靜脈的阻擋情況差異有統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)中巖靜脈的燙除情況差異有統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)中責(zé)任血管的壓迫部位差異無統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)中責(zé)任血管的類型差異無統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)后巖靜脈回流障礙相關(guān)的意識障礙差異具有統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)后其它并發(fā)癥的差異無統(tǒng)計(jì)學(xué)意義(P0.05);A、B組術(shù)后第1天、第12個(gè)月、第24個(gè)月療效差異無統(tǒng)計(jì)學(xué)意義(P0.05),但術(shù)后第36個(gè)月A、B組的療效差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:小腦裂入路術(shù)中不僅可以減少對小腦的牽拉,較好地避開巖靜脈,充分利用自然解剖間隙暴露三叉神經(jīng)根,利于責(zé)任血管的發(fā)現(xiàn)與減壓,而且術(shù)后巖靜脈回流障礙相關(guān)的并發(fā)癥少和更好的遠(yuǎn)期療效。
[Abstract]:Objective: To evaluate the cerebellar fissure approach in the trigeminal nerve micro vascular decompression in the application, to obtain more evidence and experience in clinical practice. Methods: from January 2012 -2013 December Department of neurosurgery during microvascular decompression for primary trigeminal neuralgia patients 167 cases. According to the inclusion and exclusion criteria were collected. The traditional suboccipital retrosigmoid approach group (A group) and 53 cases of cerebellar fissure approach group (B group) 61 cases, comparative analysis of two kinds of surgical procedures in the treatment of primary trigeminal neuralgia in the trigeminal nerve root exposure, petrosal vein blocking, except in cases of hot rock vein, oppression site responsible vessels, vascular type, postoperative complications and first days, twelfth months, twenty-fourth months, thirty-sixth months of treatment, and statistical analysis.
