【摘要】：背景:膝關(guān)節(jié)前交叉韌帶脛骨止點(diǎn)撕脫骨折的治療方法多樣,各有優(yōu)勢(shì)。目的:探討關(guān)節(jié)鏡下應(yīng)用兩根Arthrex線雙骨道聯(lián)合pushlock免打結(jié)錨釘固定前交叉韌帶脛骨止點(diǎn)撕脫骨折的手術(shù)方法及臨床療效。方法:納入2014年12月至2015年11月在內(nèi)蒙古醫(yī)科大學(xué)第二附屬醫(yī)院住院治療的23例前交叉韌帶脛骨止點(diǎn)撕脫骨折患者,在關(guān)節(jié)鏡下應(yīng)用Arthrex線將骨折塊十字交叉復(fù)位,在脛骨結(jié)節(jié)內(nèi)側(cè)擰入pushlock免打結(jié)錨釘以收緊Arthrex縫線,將骨折塊牢固固定于脛骨髁間嵴附著點(diǎn)處。術(shù)后采用膝關(guān)節(jié)評(píng)分(Lysholm和Tegner評(píng)分)隨訪。應(yīng)用前抽屜試驗(yàn)、Lachman試驗(yàn)評(píng)價(jià)膝關(guān)節(jié)穩(wěn)定性,X射線片評(píng)價(jià)術(shù)后骨折端的復(fù)位及愈合。結(jié)果與結(jié)論:(1)23例患者隨訪時(shí)間為6個(gè)月-1年;(2)Lysholm關(guān)節(jié)評(píng)分術(shù)前為(47.31±6.16)分,術(shù)后為(94.69±1.28)分(P0.05),Tegner評(píng)分由術(shù)前的(3.14±1.58)分提高到術(shù)后的(7.74±1.69)分(P0.05);(3)查體:Lachman試驗(yàn)均為陰性,X射線片示骨折端愈合良好。未出現(xiàn)關(guān)節(jié)反復(fù)腫脹及交鎖等機(jī)械癥狀,膝關(guān)節(jié)無(wú)屈伸活動(dòng)受限,未出現(xiàn)感染、下肢深靜脈血栓形成等其他并發(fā)癥;(4)結(jié)果表明,關(guān)節(jié)鏡下應(yīng)用Arthrex線結(jié)合pushlock免打結(jié)錨釘固定前交叉韌帶脛骨髁間嵴撕脫性骨折具有微創(chuàng)、操作簡(jiǎn)單、固定可靠、無(wú)金屬內(nèi)置物,效果滿意等優(yōu)點(diǎn)。
[Abstract]:Background: the treatment of tibial avulsion fracture of anterior cruciate ligament (ACL) of knee joint has many advantages.Objective: to investigate the operative method and clinical effect of arthroscopic fixation of tibial avulsion fracture of anterior cruciate ligament (ACL) with two Arthrex lines and two osseous pathways combined with pushlock.Methods: from December 2014 to November 2015, 23 patients with avulsion fracture of anterior cruciate ligament tibia were treated in the second affiliated Hospital of Inner Mongolia Medical University. The fracture was cross-reduced with Arthrex line under arthroscopy.The pushlock-free anchor nail was inserted into the medial tibial nodule to tighten the Arthrex suture and the fracture block was firmly fixed at the insertion point of the tibial intercondylar ridge.The knee joint score, Lysholm and Tegner score were followed up.The anterior drawer test and Lachman test were used to evaluate the stability of knee joint.There were no mechanical symptoms such as repeated swelling and locking of joints, no limited flexion and extension activity, no infection, and other complications such as deep vein thrombosis of lower extremities.Arthroscopic fixation of avulsion fracture of tibial intercondylar ridge of anterior cruciate ligament with Arthrex line and pushlock anchor nail has the advantages of minimally invasive, simple operation, reliable fixation, no metal interposition and satisfactory effect.
