【摘要】：目的研究股骨近端防旋髓內(nèi)釘(PFNA)和人工股骨頭置換術(shù)(FHPR)治療高齡骨質(zhì)疏松不穩(wěn)定股骨轉(zhuǎn)子間骨折的臨床療效。方法選取該院2010-2014年高齡嚴(yán)重骨質(zhì)疏松不穩(wěn)定型轉(zhuǎn)子間骨折患者共30例,采用區(qū)組隨機(jī)分組法將患者分成A、B兩組(各15例患者)。A組采用PFNA治療方式,B組采用FHPR治療方式。比較兩組病例術(shù)中出血量、手術(shù)時(shí)間、平均住院時(shí)間、下地活動(dòng)時(shí)間及術(shù)后6周Harris評(píng)分,觀察兩種手術(shù)方式治療老年嚴(yán)重骨質(zhì)疏松性不穩(wěn)定型股骨轉(zhuǎn)子間骨折的近期療效。結(jié)果平均失血量A組為(200.3±50.6)mL,B組為(456.2±60.2)mL,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。A組術(shù)后平均下地活動(dòng)時(shí)間為(30.2±10.1)d,B組為(14.2±7.0)d,差異有統(tǒng)計(jì)學(xué)意義(P=0.005)。結(jié)論 FHPR是治療老年性不穩(wěn)定性轉(zhuǎn)子間骨折的有效方法。
[Abstract]:Objective to study the clinical effect of proximal femoral intramedullary nail (PFNA) and artificial femoral head replacement (FHPR) in the treatment of unstable intertrochanteric fracture of the femur in elderly patients.Methods A total of 30 patients with severe unstable intertrochanteric fracture of osteoporosis were selected from 2010 to 2014. The patients were randomly divided into two groups: group A (n = 15) and group B (n = 15) were treated by PFNA. Group B was treated with FHPR.The amount of intraoperative bleeding, operative time, average hospitalization time, floor movement time and Harris score at 6 weeks after operation were compared between the two groups, and the short-term efficacy of the two surgical methods in the treatment of senile severe unstable femoral intertrochanteric fracture was observed.Conclusion FHPR is an effective method for the treatment of senile unstable intertrochanteric fracture.
