【摘要】：目的:探討60歲以上瓣膜病患者瓣膜置換術(shù)后早期死亡的危險(xiǎn)因素。方法:回顧性分析2012年4月至2016年4月新疆醫(yī)科大學(xué)第一附屬醫(yī)院收治178例年齡大于60歲老年瓣膜病患者行心臟瓣膜置換術(shù)的臨床資料,其中男102例,女76例;年齡60~74歲,術(shù)前經(jīng)體格檢查,實(shí)驗(yàn)室檢查及其他綜合評定后確診,均經(jīng)手術(shù)證實(shí),均在全麻體外循環(huán)(cardiopulmonary bypass,CPB)下行心臟瓣膜置換術(shù)。結(jié)果:178例患者中術(shù)后發(fā)生嚴(yán)重并發(fā)癥17例(9.5%),死亡10例(5.61%)。臨床通過統(tǒng)計(jì)學(xué)單因素分析可知,CPB時(shí)間≥120min、主動(dòng)脈阻斷時(shí)間≥60min、有并發(fā)癥、EF50%、LVEDD≥70mm和同期行CABG術(shù)死亡的概率更高,多因素Logistic回歸分析顯示,體外循環(huán)時(shí)間≥120min,EF50%;老年心臟瓣膜患者同期行CABG術(shù)及有并發(fā)癥是早期死亡的獨(dú)立預(yù)測因素。結(jié)論:根據(jù)老年心臟瓣膜病患者的臨床特點(diǎn)進(jìn)行術(shù)前評估,針對性的給予老年心臟瓣膜病患者瓣膜置換術(shù)治療有助于延長患者的生存時(shí)間,提高患者的生存率,改善患者的預(yù)后。
[Abstract]:Objective: to investigate the risk factors of early death after valve replacement in patients over 60 years old. Methods: a retrospective analysis of 178 patients aged over 60 years in the first affiliated Hospital of Xinjiang Medical University from April 2012 to April 2016 was conducted. Clinical data of valve replacement in patients with valvular disease, 102 males and 76 females, aged 60 to 74 years, were diagnosed by physical examination, laboratory examination and other comprehensive assessment before operation, all of which were confirmed by operation.
