【摘要】：目的:觀(guān)察超聲引導(dǎo)肌間溝注射治療頸部疼痛相關(guān)性眩暈的臨床療效,探討頸部疼痛在眩暈發(fā)病機(jī)制中的作用。方法:采用隨機(jī)數(shù)字表法將40例頸性眩暈患者分為治療組及對(duì)照組。兩組患者均給予常規(guī)治療,包括改善后循環(huán)藥物、心理治療;治療組患者在此基礎(chǔ)上給予超聲引導(dǎo)肌間溝注射治療。于治療前、治療2周后采用頸交感神經(jīng)癥狀評(píng)分表(cervical symphathetic symptoms evaluation questionnaire,CSSEQ)對(duì)兩組患者進(jìn)行療效評(píng)定,同時(shí)記錄治療組超聲引導(dǎo)肌間溝注射治療主觀(guān)感受評(píng)價(jià)、眩暈改善起效時(shí)間、眩暈消失時(shí)間。結(jié)果:兩組間治療前CSSEQ評(píng)分比較,差異無(wú)顯著性意義(P0.05),治療2周后治療組CSSEQ評(píng)分(5.26±2.47)分、對(duì)照組(12.84±3.71)分,均較治療前明顯降低(P0.05),治療組較對(duì)照組評(píng)分降低更為明顯(P0.05);治療組注射后0.5h內(nèi)所有患者治療側(cè)頸肩部脹、熱,上肢麻木、乏力,18例患者自覺(jué)頭清目明、視物清晰。治療組眩暈改善起效時(shí)間平均為注射后(4.0±1.5)h,眩暈消失時(shí)間平均為(5.0±1.5)d。結(jié)論:定位精準(zhǔn)的超聲引導(dǎo)肌間溝注射治療頸部疼痛相關(guān)性眩暈有效,消除頸部疼痛是治療頸性眩暈的關(guān)鍵。
[Abstract]:Objective: to observe the clinical effect of ultrasound-guided intramuscular groove injection in the treatment of cervical pain associated vertigo. Methods: 40 patients with cervical vertigo were randomly divided into treatment group and control group. The patients in the treatment group were treated with ultrasound-guided intramuscular sulcus injection. Before and 2 weeks after treatment, the patients in the two groups were evaluated with cervical symphathetic symptoms evaluation questionnaire CSSEQ. At the same time, the subjective feeling evaluation of ultrasound guided intramuscular sulcus injection was recorded in the treatment group, the onset time of vertigo was improved and the time of vertigo disappeared. Results: CSSEQ scores were compared before treatment between the two groups. After 2 weeks of treatment, the CSSEQ score of the treatment group was 5.26 鹵2.47, and that of the control group was 12.84 鹵3.71, which was significantly lower than that of the control group (P 0.05), and that of the treatment group was significantly lower than that of the control group (P 0.05), and that of the treatment group was significantly lower than that of the control group (P 0.05), and the neck and shoulder distended and hot in all the patients in the treatment group within 0.5 h after injection. 18 patients with upper limb numbness and fatigue were conscious of clearing their head and eyes. Visual clarity. The average improvement time of vertigo in treatment group was 4.0 鹵1.5 hours after injection, and the average disappearance time of vertigo was 5.0 鹵1.5 days. Conclusion: ultrasonic guided intramuscular groove injection is effective in treating cervical pain related vertigo. Eliminating neck pain is the key to the treatment of cervical vertigo.
