【摘要】：目的探討肘關(guān)節(jié)恐怖三聯(lián)征患者圍術(shù)期激素的應用對其術(shù)后疼痛、運動功能以及并發(fā)癥的影響。方法 17例圍術(shù)期應用激素的患者為激素組,17例無激素用藥史的患者為對照組,激素組術(shù)中給予地塞米松10 mg靜脈推注,術(shù)后6 d逐漸減量口服甲強龍。兩組術(shù)后均前臂旋前位屈肘90°石膏外固定2 w后拆除外固定開始行肘關(guān)節(jié)功能鍛煉,在術(shù)后的2、6、12、24 w分別記錄并評估各組患者的疼痛評分、肘關(guān)節(jié)活動度及各種并發(fā)癥的發(fā)生情況。結(jié)果術(shù)后24 w時激素組比對照組患肢的屈伸角度更大(P0.05),24 w前的隨訪中兩組數(shù)據(jù)無顯著差異(P0.05)。所有時間點激素組的前臂旋后角度均大于對照組(P0.05)。對照組有4種并發(fā)癥,激素組有2種。最常見的并發(fā)癥是異位骨化和尺神經(jīng)麻痹,所有的異位骨化均發(fā)生在對照組,其中有1例需要手術(shù)干預。兩組均無術(shù)后感染、骨折不愈合或延遲愈合發(fā)生。結(jié)論恐怖三聯(lián)征患者圍術(shù)期激素的應用有助于其術(shù)后肘關(guān)節(jié)的屈伸旋轉(zhuǎn)功能的恢復,尤其是可以提高患肢的旋后功能,且激素的應用并不會增加患者術(shù)后的疼痛評分及感染發(fā)生率。
[Abstract]:Objective to investigate the effect of perioperative hormone application on postoperative pain, motor function and complications in patients with triple syndrome of elbow joint terror. Methods 17 patients with perioperative hormone use were treated with hormone group and 17 patients with no history of hormone use as control group. In the steroid group, dexamethasone 10 mg was given intravenously, and oral methylenolone was gradually reduced 6 days after operation. After 2 weeks of external fixation of 90 擄anteflexion of forearm, the external fixation was removed and the function of elbow joint was performed. Pain scores were recorded and evaluated 24 weeks after operation. Results at 24 weeks after operation, the angle of flexion and extension of the affected limbs in the hormone group was greater than that in the control group. There was no significant difference between the two groups in the data of the two groups at 24 weeks after operation, and there was no significant difference between the two groups at all time points in the forearm rotation of the hormone group. The posterior angle was higher than that in the control group (P 0.05). There were 4 kinds of complications in the control group. The most common complications were ectopic ossification and ulnar nerve palsy. All ectopic ossification occurred in the control group. Conclusion the perioperative hormone application in the patients with terrors syndrome is helpful to the recovery of elbow flexion and extension rotation function, especially to the improvement of circumflex function of the affected limbs. And the use of hormones did not increase postoperative pain score and incidence of infection.
