【摘要】：目的 :探討腰椎融合術(shù)后早期下肢深靜脈血栓(deep venous thrombosis,DVT)形成的危險因素。方法:選取于2015年7月~2017年5月在廣西壯族自治區(qū)人民醫(yī)院骨科接受腰椎后路椎間融合術(shù)治療的患者共165例,依據(jù)術(shù)后早期超聲檢查結(jié)果雙下肢有無血栓形成,分為DVT組和非DVT組。對比兩組患者的性別、年齡、體重指數(shù)、糖尿病、高血壓病、術(shù)前凝血四項、D-二聚體、融合節(jié)段數(shù)、手術(shù)時間、術(shù)中輸血、術(shù)后使用抗凝藥、術(shù)后臥床時間等指標(biāo),再采用Logistic多因素回歸分析進(jìn)行統(tǒng)計學(xué)分析。結(jié)果:共納入165例患者,術(shù)后并發(fā)下肢DVT者24例,發(fā)生率為14.5%。與非DVT組相比較,單因素χ~2檢驗顯示兩組在年齡、體重指數(shù)、手術(shù)時間、術(shù)中輸血、術(shù)后臥床時間等指標(biāo)差異具有統(tǒng)計學(xué)意義。多因素Logistic回歸分析顯示,年齡≥60歲(OR=6.444,95%CI=2.116~19.628)、體重指數(shù)≥24kg/m~2(OR=4.463,95%CI=1.603~12.426)、術(shù)中輸血(OR=3.484,95%CI=1.110~10.932)、術(shù)后臥床天數(shù)≥5d(OR=3.155,95%CI=1.027~9.695)是腰椎融合術(shù)后早期并發(fā)下肢DVT的獨立危險因素。結(jié)論:年齡≥60歲、體重指數(shù)≥24kg/m2、術(shù)中輸血、術(shù)后臥床天數(shù)≥5d的患者腰椎融合術(shù)后早期易出現(xiàn)下肢DVT形成。
[Abstract]:Objective: to investigate the risk factors of early deep venous thromboembolism (DVT) of lower extremity after lumbar fusion. Methods: posterior lumbar interbody fusion was performed from July 2015 to May 2017 in the Department of Orthopaedics, Guangxi Zhuang Autonomous region people's Hospital. A total of 165 patients were treated with surgery. According to the results of early postoperative ultrasound, the patients were divided into DVT group and non-#en1# group. The sex, age, body mass index, diabetes mellitus, hypertension, four items of D-dimer, and the number of fusion segments were compared between the two groups. The time of operation, blood transfusion during operation, anticoagulant and bed rest after operation were analyzed by Logistic multivariate regression analysis. Results: among the 165 patients, 24 patients were complicated with lower extremity DVT after operation. The incidence rate was 14.5.Compared with the non-#en0# group, univariate 蠂 ~ 2 test showed that there were significant differences in age, body mass index, operation time, blood transfusion during operation and bed rest time after operation between the two groups.
