【摘要】：目的探討一種改良拇指橈側(cè)筋膜蒂皮瓣修復拇指軟組織缺損的臨床療效。方法 2015年6月—2016年12月,采用筋膜蒂遠端切斷的改良拇指橈側(cè)筋膜蒂皮瓣,修復15例拇指掌背側(cè)軟組織缺損患者。其中男11例,女4例;年齡35~70歲,平均46歲。致傷原因:擠壓傷12例,撕脫傷3例。均為拇指掌背側(cè)軟組織缺損伴肌腱或骨組織外露,無再植條件或再植欲望。拇指掌側(cè)缺損12例,背側(cè)缺損3例;創(chuàng)面缺損范圍為2.0 cm×1.2 cm~3.0 cm×2.5 cm。受傷至手術(shù)時間為16 h~2 d,平均30.4 h。皮瓣切取范圍2.3 cm×1.5 cm~3.3 cm×2.8 cm;供區(qū)均直接縫合。結(jié)果術(shù)后皮瓣均成活,無極度腫脹和張力性水皰發(fā)生;供區(qū)及創(chuàng)面均Ⅰ期愈合。15例患者均獲隨訪,隨訪時間3~12個月,平均6個月。筋膜蒂皮瓣色澤與周圍組織相近,無明顯臃腫,外觀滿意。末次隨訪時按照總主動活動度法評定手功能,獲優(yōu)8例,良7例。結(jié)論采用改良拇指橈側(cè)筋膜蒂皮瓣修復拇指軟組織缺損具有損傷小、手術(shù)操作簡便的優(yōu)點,同時可獲得可靠的臨床療效。
[Abstract]:Objective to investigate the clinical effect of a modified radial fasciocutaneous flap of thumb for the repair of thumb soft tissue defect. Methods from June 2015 to December 2016, the modified radial fasciocutaneous flap with distal fascia pedicle was used. 15 cases of soft tissue defect of dorsal palm of thumb were repaired. There were 11 males and 4 females, aged 3570 years, with an average age of 46 years. The causes of injury were crush injury in 12 cases and avulsion injury in 3 cases. No replantation condition or replantation desire. Palmar defect of thumb in 12 cases, dorsal defect in 3 cases; The area of wound defect was 2.0 cm 脳 1.2 cm~3.0 cm 脳 2.5 cm, the average time from injury to operation was 30.4 h. The skin flap was cut at 2.3 cm 脳 1.5 cm~3.3 cm 脳 2.8 cm, and the donor area was sutured directly. Results the flap survived without swelling and tension blister.
