【摘要】：背景:目前評價(jià)股骨頭壞死病灶體積大小的影像學(xué)方法有基于X射線的Kerboul角、壞死指數(shù),基于CT、MRI圖像三維重建計(jì)算股骨頭壞死體積等,但各方法的準(zhǔn)確度報(bào)道不一,尚無一種被廣泛接受;同時(shí),并沒有研究明確揭示骨壞死病變范圍、程度與股骨頭血供損害的直接聯(lián)系。目的:評估各種常用影像技術(shù)在不同時(shí)期骨壞死病灶評價(jià)中的準(zhǔn)確性,嘗試確立一種更加簡便易用的骨壞死病灶臨床計(jì)算方法。方法:實(shí)驗(yàn)為診斷性動(dòng)物體內(nèi)實(shí)驗(yàn),在中國大連,大連大學(xué)附屬中山醫(yī)院完成。制備激素性股骨頭壞死犬模型,通過MRI、CT檢查,結(jié)合Micro-CT離體標(biāo)本檢測對壞死股骨頭病變進(jìn)行精確評估,結(jié)合血管灌注造影及病理檢測評價(jià)骨壞死發(fā)生范圍、骨破壞程度與血供變化的關(guān)聯(lián);同時(shí)將結(jié)果與臨床常用僅基于影像學(xué)的骨壞死分期分級標(biāo)準(zhǔn)相比較,以期驗(yàn)證此精確測量方法與傳統(tǒng)方法的一致性。實(shí)驗(yàn)過程遵循了國際獸醫(yī)學(xué)編輯協(xié)會(huì)《關(guān)于動(dòng)物倫理與福利的作者指南共識》和本地及國家法規(guī),并遵守《動(dòng)物實(shí)驗(yàn)體內(nèi)實(shí)驗(yàn)研究報(bào)告規(guī)范指南》(ARRIVE指南)。結(jié)果與結(jié)論:課題擬建立針對骨壞死不同分期采用不同影像學(xué)技術(shù)測定以獲得準(zhǔn)確結(jié)果的新理念,將使不同技術(shù)能力的醫(yī)療機(jī)構(gòu)能夠應(yīng)用獲得的影像資料給出相對更準(zhǔn)確的骨壞死評估結(jié)果,使骨壞死病變分期評價(jià)更準(zhǔn)確,進(jìn)而患者有望得到針對評估結(jié)果的標(biāo)準(zhǔn)化治療方案。
[Abstract]:Background: at present, the imaging methods for evaluating the volume of femoral head necrosis focus include X-ray based Kerboul angle, necrosis index, 3D reconstruction of femoral head necrosis volume and so on. However, the accuracy of these methods is not consistent. At the same time, there is no clear relationship between the extent and extent of osteonecrosis and the blood supply of femoral head. Objective: to evaluate the accuracy of various imaging techniques in the evaluation of osteonecrosis lesions at different stages. This paper attempts to establish a simple and easy to use clinical calculation method for osteonecrosis lesions. Methods: a canine model of steroid-induced osteonecrosis was established in the Zhongshan Hospital affiliated to Dalian University in Dalian, China. The lesions of the necrotic femoral head were accurately evaluated by MRII-CT and Micro-CT in vitro. The extent of osteonecrosis, the degree of bone destruction and the changes of blood supply were evaluated by angiography and pathological examination. At the same time, the results were compared with the clinical osteonecrosis classification standard based only on imaging. In order to verify the consistency of this accurate measurement with traditional methods, the experiment followed the International Veterinary editorial Association's consensus on the authors' Guide to Animal Ethics and Welfare and local and national regulations. Results and conclusion: a new concept of different imaging techniques for different stages of osteonecrosis was established to obtain accurate results. Medical institutions with different technical abilities will be able to use the obtained image data to give a more accurate evaluation result of osteonecrosis, so that the stage evaluation of osteonecrosis lesions will be more accurate, and the patients will be expected to obtain a standardized treatment plan for the evaluation results.
