【摘要】：[目的]分析胸腰段脊柱骨折患者發(fā)生醫(yī)院感染的影響因素并提出防治措施。[方法]回顧2010年1月~2015年12月264例胸腰段脊柱骨折患者的臨床資料,收集并逐一記錄每位患者的性別、年齡、TLICS評(píng)分、是否有機(jī)械通氣和保留導(dǎo)尿等侵入治療、采取保守或手術(shù)治療方式、是否有復(fù)合傷、入住ICU、住院天數(shù)、合并甚而基礎(chǔ)疾病與臥床時(shí)間等臨床資料,對(duì)其醫(yī)院感染的影響因素進(jìn)行分析。[結(jié)果]單因素分析結(jié)果顯示患者年齡大、TLICS評(píng)分高、侵入性操作、入住ICU、住院時(shí)間(d)、合并基礎(chǔ)疾病、臥床時(shí)間長(zhǎng)(d)和使用糖皮質(zhì)激素≥3(d)等8個(gè)因素均是醫(yī)院感染的危險(xiǎn)因素(P0.05),手術(shù)治療是醫(yī)院感染保護(hù)性因素(P0.05);logistic回歸分析顯示其中年齡(OR1.050,95%C1.006~1.095)、TLICS評(píng)分高(OR4.465,95%1.949~10.230)、侵入性操作(OR11.849,95%CI3.302~42.523)和臥床時(shí)間(d)(OR1.368,95%CI1.215~1.541)等4個(gè)因素是獨(dú)立危險(xiǎn)因素。[結(jié)論]胸腰段脊柱骨折患者發(fā)生醫(yī)院感染受多種因素影響,應(yīng)針對(duì)相關(guān)危險(xiǎn)因素特別是獨(dú)立危險(xiǎn)因素采取防控,增加保護(hù)性措施,減少醫(yī)院感染發(fā)生。
[Abstract]:[objective] to analyze the influencing factors of nosocomial infection in patients with thoracolumbar spinal fractures and to put forward the preventive and therapeutic measures. [methods] the clinical data of 264 patients with thoracolumbar spinal fractures from January 2010 to December 2015 were reviewed. Each patient's gender, age and TLICS scores were collected and recorded one by one. Whether there were mechanical ventilation or retention catheterization, conservative or surgical treatment, compound injury, ICU, hospital stay, etc. The factors influencing the nosocomial infection were analyzed. [results] the univariate analysis showed that the patients had high TLICS score and invasive operation.
