【摘要】：目的通過與傳統(tǒng)縫合法比較,探討組織膠水用于人工全髖關(guān)節(jié)置換術(shù)(total hip arthroplasty,THA)的可行性及療效。方法將2015年1月-10月收治并符合選擇標(biāo)準(zhǔn)的159例初次THA患者納入研究,其中80例(100髖)術(shù)中采用可吸收縫合線及組織膠水閉合切口(試驗組);79例(100髖)采用傳統(tǒng)皮釘閉合切口(對照組)。兩組患者性別、年齡、體質(zhì)量指數(shù)、病因等一般資料比較,差異均無統(tǒng)計學(xué)意義(P0.05)。記錄并比較兩組手術(shù)時間、住院時間,術(shù)后15 d行切口疼痛視覺模擬評分(VAS)、切口愈合評分、切口愈合分級及切口滿意度評價。結(jié)果兩組患者均獲隨訪,隨訪時間3~6個月,平均4.7個月。試驗組手術(shù)時間與對照組相比,差異無統(tǒng)計學(xué)意義(Z=-0.527,P=0.598);但住院時間較對照組縮短(Z=-2.004,P=0.045)。術(shù)后試驗組1例(1髖)發(fā)生切口滲出,1例(1髖)切口裂開;對照組2例(2髖)發(fā)生切口周圍紅腫,2例(2髖)切口持續(xù)滲出,4例(4髖)引流口加壓包扎后仍有滲液,1例(1髖)誤縫引流管。術(shù)后15 d兩組VAS評分比較差異無統(tǒng)計學(xué)意義(Z=-0.125,P=0.901);但試驗組切口愈合評分顯著高于對照組(Z=-2.943,P=0.003)。試驗組切口甲級愈合98髖、乙級愈合2髖,對照組分別為92髖、8髖,比較差異有統(tǒng)計學(xué)意義(χ~2=3.789,P=0.048)。試驗組切口滿意度評分顯著高于對照組(Z=-2.626,P=0.009)。結(jié)論與傳統(tǒng)縫合法相比,初次THA術(shù)中應(yīng)用組織膠水閉合切口,術(shù)后切口并發(fā)癥發(fā)生率低,且操作簡便、安全,患者滿意度高。
[Abstract]:Objective to explore the feasibility and curative effect of tissue glue in total hip arthroplasty by comparing with traditional suture. Methods from January 2015 to October, 159 primary THA patients who met the selection criteria were included in the study. 80 cases (100 hips) were treated with absorbable suture and tissue glue closed incision (79 cases with 100 hips in the experimental group) with traditional skin nail incision (control group). The general data of sex, age, body mass index, etiology and so on were compared between the two groups. There was no significant difference between the two groups (P 0.05). The operation time, hospital stay, visual analogue score of incision pain, wound healing score, wound healing grade and wound satisfaction were recorded and compared 15 days after operation. Results the patients in both groups were followed up. The follow-up time was 3 ~ 6 months (mean 4.7 months). Compared with the control group, the operation time of the trial group was not significantly different from that of the control group, but the hospitalization time was shorter than that of the control group, but the hospitalization time was shorter than that in the control group. In the control group, 2 cases (2 hips) developed redness around the incision (2 cases (2 hips)) continuous exudation of incision (4 cases (4 hips)) after compression bandaging of drainage orifice, there was still exudate in 1 case (1 hip) missewed drainage tube. There was no difference in VAS score between the two groups 15 days after operation. The score of wound healing in the experimental group was significantly higher than that in the control group (P < 0.05), but the score of wound healing in the experimental group was significantly higher than that in the control group (n = 98), but the score of wound healing in the experimental group was significantly higher than that in the control group (n = 98). Two hips healed in grade B and 92 hips in control group (蠂 ~ 2 / 789), and the difference was statistically significant (蠂 ~ (2 / 2) 3.789). The score of incision satisfaction in the experimental group was significantly higher than that in the control group. Conclusion compared with the traditional suture method, the first time THA operation was performed with glue closed incision, and the wound satisfaction score was significantly higher than that in the control group (P < 0.05). The incidence of postoperative incision complications was low, the operation was simple, safe, and the patient satisfaction was high.
