3D打印與虛擬手術(shù)設(shè)計(jì)在復(fù)雜脛骨平臺(tái)骨折手術(shù)治療中的應(yīng)用

【摘要】:目的探討應(yīng)用3D打印技術(shù)結(jié)合虛擬手術(shù)設(shè)計(jì)內(nèi)固定治療復(fù)雜脛骨平臺(tái)骨折的療效。方法筆者自2013-01—2014-12診治復(fù)雜脛骨平臺(tái)骨折45例,按入院先后順序的奇偶性分為A組(應(yīng)用3D打印虛擬手術(shù))和B組(應(yīng)用常規(guī)手術(shù))。A組術(shù)前采用三維重建、虛擬復(fù)位、3D打印及個(gè)性化模型預(yù)手術(shù)。比較2組手術(shù)時(shí)間及鋼板一次性放置率、復(fù)位滿意度、膝關(guān)節(jié)功能等。結(jié)果所有患者獲得隨訪6~30個(gè)月,平均16.8個(gè)月。2組切開顯露時(shí)間差異無統(tǒng)計(jì)學(xué)意義(P0.05);A組在復(fù)位固定時(shí)間上低于B組,2組鋼板一次性放置率(χ~2=7.238,P=0.007)、復(fù)位滿意度(χ~2=4.499,P=0.034)比較,差異有統(tǒng)計(jì)學(xué)意義;A組術(shù)后HSS評(píng)分優(yōu)良率91%,高于B組優(yōu)良率77%,2組優(yōu)良率比較差異無統(tǒng)計(jì)學(xué)意義(χ~2=1.658,P=0.194)。結(jié)論 3D打印結(jié)合虛擬手術(shù)內(nèi)固定可提升脛骨平臺(tái)骨折整體手術(shù)效果。
[Abstract]:Objective to investigate the curative effect of the combined application of 3D printing technology, virtual surgery design of internal fixation for the treatment of complex tibial plateau fractures. Methods the author from 2013-01 - 2014-12 in diagnosis and treatment of complex tibial plateau fracture 45 cases, according to the parity of the admission order divided into group A (Application of 3D printing and B (virtual surgery group) using conventional surgery).A group before operation by 3D reconstruction, virtual reset, 3D printing and personalized model of pre operation. The 2 groups were compared in operation time and plate placed reduction rate, satisfaction, knee joint function. Results all patients were followed up for 6~30 months, average 16.8 months in group.2 incision exposure time had no significant difference (P0.05); A group was lower than that of B in the group of fixation time, 2 groups of plate placed rate (x ~2=7.238, P=0.007), reset satisfaction (x ~2=4.499, P=0.034), the difference was statistically significant; group A postoperative HSS score excellent rate of 91%, higher than that of B group. The good rate was 77%. There was no significant difference in the excellent and good rate between the 2 groups (chi ~2=1.658, P=0.194). Conclusion 3D printing combined with virtual surgery internal fixation can improve the operative effect of tibial plateau fracture.
