【摘要】：目的探討股骨近端鎖定鋼板聯(lián)合自體髂骨結構性植骨治療股骨距粉碎性股骨轉子間骨折的臨床效果。方法選擇45例股骨距粉碎性股骨轉子間骨折患者,采用股骨近端鎖定鋼板結合自體髂骨結構性植骨手術治療,根據(jù)Harris髖關節(jié)評分系統(tǒng)評價患髖功能。結果所有病例均在門診獲得隨訪,隨訪時間8~34個月,平均16個月。無骨折不愈合、復位丟失、髖內(nèi)翻、內(nèi)固定松動、斷裂或切出、股骨頭缺血性壞死、肢體短縮等并發(fā)癥發(fā)生。參照Harris髖關節(jié)評分系統(tǒng)評定患髖功能,優(yōu)29例,良11例,中4例,差1例,優(yōu)良率為88.9%。結論采用自體髂骨結構性植骨重建粉碎、缺損的股骨距可加強內(nèi)固定效果,促進骨折愈合,聯(lián)合股骨近端鎖定鋼板效果更好。
[Abstract]:Objective to investigate the clinical effect of proximal locking plate combined with autogenous iliac bone graft in the treatment of comminuted intertrochanteric fracture of femur. Methods 45 patients with comminuted femoral intertrochanteric fracture were selected. Femoral proximal locking plate combined with autogenous iliac bone graft was used to evaluate hip function according to Harris hip scoring system. Results all the patients were followed up in outpatient department for 8 ~ 34 months. There were no complications such as nonunion of fracture, loss of reduction, coxa varus, loosening of internal fixation, rupture or excision, avascular necrosis of femoral head and short contraction of extremities, etc. According to the Harris hip joint scoring system, the hip function was excellent in 29 cases. 11 cases were good, 4 cases were moderate, 1 case was poor, the excellent and good rate was 88.9.Conclusion using autogenous iliac bone structural bone graft to reconstruct comminution, the defect of femur distance can strengthen the effect of internal fixation, promote fracture healing, and combine with locking plate of proximal femur is better.
