3D打印技術(shù)輔助股骨遠端截骨術(shù)治療膝外翻畸形骨關(guān)節(jié)炎的療效觀察

【摘要】:目的探討3D打印技術(shù)輔助股骨遠端截骨術(shù)(distal femoral osteotomy,DFO)治療膝外翻畸形骨關(guān)節(jié)炎的臨床療效。方法2014年1月—2016年1月,收治膝外翻畸形骨關(guān)節(jié)炎患者12例(15膝)。男5例,女7例;年齡30~60歲,平均43.8歲。病程1~12年,平均6.6年。單膝9例,雙膝3例。骨關(guān)節(jié)炎Koshino分級:Ⅰ級1例(1膝),Ⅱ級7例(9膝),Ⅲ級4例(5膝)。攝雙下肢全長X線片,測量股脛角(femorotibial angle,FTA)為(160.40±2.69)°,遠端股骨外側(cè)角(anatomical lateral distal femoral angle,a LDFA)為(64.20±2.11)°。術(shù)前于Mimics軟件設(shè)計截骨模塊并3D打印,術(shù)中通過截骨模塊確定最佳截骨方位進行截骨,最后行鋼板及螺釘內(nèi)固定。結(jié)果術(shù)后切口均Ⅰ期愈合,無感染、下肢深靜脈血栓形成等并發(fā)癥發(fā)生;颊呔@隨訪,隨訪時間6~18個月,平均12.2個月。術(shù)后6個月膝關(guān)節(jié)HSS評分為(89.07±2.49)分,較術(shù)前(65.27±1.49)分顯著提高(t= 28.31,P=0.00);獲優(yōu)10膝、良4膝、可1膝,優(yōu)良率為93.3%。X線片復(fù)查示,患者截骨處均達骨性愈合,愈合時間2.9~4.8個月,平均3.3個月;1例(1膝)出現(xiàn)骨延遲愈合。術(shù)后6個月測量FTA為(174.00±1.41)°,a LDFA為(81.87±1.06)°,與術(shù)前比較差異均有統(tǒng)計學(xué)意義(t= 18.26,P=0.00;t= 25.19,P=0.00);下肢力線位置測量顯示內(nèi)側(cè)脛骨平臺占整個脛骨平臺百分比為49.78%±0.59%,與術(shù)中測量的49.82%±0.77%比較,差異無統(tǒng)計學(xué)意義(t=0.14,P=0.89)。結(jié)論DFO治療膝外翻畸形骨關(guān)節(jié)炎時,3D打印截骨模塊可輔助術(shù)中股骨遠端精確截骨,獲得滿意臨床療效。
[Abstract]:Objective to investigate the clinical effect of 3D printing assisted distal femur osteotomy in the treatment of osteoarthritis with valgus deformity. Methods from January 2014 to January 2016, 12 patients with osteoarthritis of valgus deformity were treated, including 5 males and 7 females. The average age was 43.8 years.
