【摘要】：目的比較直接前方入路與傳統(tǒng)后外側(cè)入路行人工全髖關(guān)節(jié)置換術(shù)后髖臼假體位置的差異。方法將2008年12月—2015年12月收治并符合選擇標(biāo)準(zhǔn)的102例行人工全髖關(guān)節(jié)置換術(shù)患者納入研究;置換術(shù)中采用直接前方入路51例(直接前方入路組)、后外側(cè)入路51例(后外側(cè)入路組)。兩組患者性別、年齡、體質(zhì)量指數(shù)、髖別、病因等一般資料比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05),具有可比性。于術(shù)后第1天患者骨盆正位X線片測(cè)量髖臼外展角及前傾角,并根據(jù)以上兩指標(biāo)評(píng)價(jià)髖臼假體是否位于安全區(qū)域。結(jié)果直接前方入路組髖臼外展角和前傾角分別為(42.28±5.77)、(21.14±5.17)°,后外側(cè)入路組分別為(43.93±7.44)、(21.05±4.10)°,比較差異均無統(tǒng)計(jì)學(xué)意義(t=1.30,P=0.19;t=0.05,P=0.96)。直接前方入路組髖臼外展角、前傾角位于安全區(qū)域比例分別為88.2%(45/51)、80.4%(41/51),后外側(cè)入路組為84.3%(43/51)、82.4%(42/51),比較差異均無統(tǒng)計(jì)學(xué)意義(χ~2=0.33,P=0.56;χ~2=0.06,P=0.79)。直接前方入路組髖臼外展角及前傾角均在安全區(qū)域的比例為70.6%(36/51),后外側(cè)入路組為68.6%(35/51),比較差異無統(tǒng)計(jì)學(xué)意義(χ~2=0.05,P=0.82)。結(jié)論采用直接前方入路或后外側(cè)入路行人工全髖關(guān)節(jié)置換術(shù),髖臼假體位置無明顯差異。
[Abstract]:Objective to compare the position of acetabular prosthesis between direct anterior approach and traditional posterolateral approach after total hip replacement. Methods 102 cases of total hip prosthesis were treated from December 2008 to December 2015 and met the selection criteria. Patients undergoing surgery were included in the study; Direct anterior approach was used in 51 cases (direct anterior approach group, posterior lateral approach group 51 cases). Sex, age, body mass index, hip difference, etiology and other general data were compared between the two groups. The differences were not statistically significant (P 0.05) and comparable. The acetabular abduction angle and anteversion angle were measured on the first day after operation. Results the acetabular abduction angle and anteversion angle in the direct anterior approach group were 42.28 鹵5.77 and 21.14 鹵5.17 擄, respectively, and the posterior lateral approach group were 43.93 鹵7.44 and 21.05 鹵4.10 擄, respectively. There was no significant difference between the two groups. Acetabular outreaching angle, The ratio of anterior inclination to safe area was 88.2R / 51N / 80.4 / 41 / 51g, respectively, and that of posterior lateral approach group was 84.343 / 51g / 42 / 51g respectively. There was no significant difference between the two groups (蠂 ~ (20.33) P ~ (0.56); 蠂 ~ (20.06) P ~ (0.79) P ~ (0.79)). The ratio of acetabular extroversion angle and anterior inclination in the direct forward approach group was 70.636 / 51g, and that in the posterolateral approach group was 68.663 / 3551g / g, and the ratio of acetabular angle and anterior inclination was 70.636 / 51g / 51g, respectively (蠂 ~ (20.33) P ~ (0.56) P ~ (0.56); 蠂 ~ (20.06) P ~ (0.79) P ~ (0.79)). There was no significant difference between the two groups (蠂 2 / 0. 05). Conclusion the direct anterior approach or posterolateral approach is used for total hip replacement. There was no significant difference in the position of acetabular prosthesis.
