【摘要】：目的據(jù)膝關(guān)節(jié)MRI結(jié)合半月板周緣性脫位移位指數(shù)統(tǒng)計(jì)半月板周緣性脫位的發(fā)生率及相關(guān)影響因素,探討其臨床意義。方法回顧性分析2015年10月至2016年3月共201例因膝關(guān)節(jié)疾病在我科住院患者的膝關(guān)節(jié)MRI。以膝關(guān)節(jié)股骨和脛骨關(guān)節(jié)面內(nèi)外側(cè)緣連線或脛骨平臺(tái)前緣為基線,半月板滑膜緣突出部分寬度超過同層面自體寬度的25%認(rèn)定存在半月板周緣性脫位。結(jié)果分析發(fā)現(xiàn)男女之間(χ2=10.745,P=0.001),內(nèi)外側(cè)半月板之間(χ2=79.90,P0.001),不同年齡組患者(50歲,≥50歲)之間半月板周緣性脫位的發(fā)生率比較差異具有統(tǒng)計(jì)學(xué)意義(χ2=24.616,P0.001),身體質(zhì)量指數(shù)(body mass index,BMI)不同(25kg/m2,≥25kg/m2)的兩組患者之間半月板周緣性脫位的發(fā)生率比較差異無統(tǒng)計(jì)學(xué)意義(χ2=0.248,P=0.619),半月板體部周緣性脫位的發(fā)生率與前角和前角+體部的發(fā)生率相比差異具有統(tǒng)計(jì)學(xué)意義(χ2=46.391,P0.001)。經(jīng)卡方分割法比較,半月板體部脫位的發(fā)生率較高。經(jīng)Logistic回歸分析,女性(比值比2.605,95%可信區(qū)間1.412~4.807)和年齡(比值比1.051,95%可信區(qū)間1.022~1.081)可能是半月板周緣性脫位的危險(xiǎn)因素。結(jié)論女性和年齡偏大可能是半月板周緣性脫位的危險(xiǎn)因素,BMI與半月板周緣性脫位的發(fā)生無相關(guān)性,內(nèi)側(cè)半月板較外側(cè)半月板周緣性脫位的發(fā)生率高,半月板體部較前角和前角+體部的發(fā)生率高。
[Abstract]:Objective to analyze the incidence and related factors of peripheral dislocation of meniscus according to MRI and displacement index of meniscus. Methods from October 2015 to March 2016, a total of 201 cases of knee joint MRIs in our department were analyzed retrospectively. The internal and external lateral line of the femoral and tibial articular surface or the front edge of the tibial plateau were used as the baseline. 25% of the meniscus synovial margin protrusions whose width exceeded the autologous width of the meniscus were found to have peripheral dislocation of meniscus. Results the results showed that there were 50 years old meniscus between male and female (蠂 2 + 10.745) and between lateral and lateral meniscus (蠂 ~ 2 / 79.90 / P ~ (0.001), 蠂 ~ 2 / 79.90 / P ~ (0.001)).
