多層螺旋CT三維重建評(píng)價(jià)“三維平衡正脊手法”治療神經(jīng)根型頸椎病療效的臨床研究

【摘要】:目的運(yùn)用多層螺旋CT掃描技術(shù),通過三維重建分析神經(jīng)根型頸椎病患者“三維定點(diǎn)平衡正脊”治療前后C4~C7段力學(xué)模型變化,評(píng)價(jià)三維平衡正脊手法治療神經(jīng)根型頸椎病的療效,探討其作用機(jī)理。方法30例符合神經(jīng)根型頸椎病診斷標(biāo)準(zhǔn)的患者,采用三維平衡正脊手法治療前后分別行頸椎螺旋CT掃描,并三維重建頸椎立體模型。行三維平衡正脊手法治療一療程后,分析螺旋CT觀察指標(biāo)的變化,包括:頸椎生理曲度、頸椎棘突與中線的關(guān)系、椎間孔、頸椎間盤的變化情況,并結(jié)合癥狀、體征進(jìn)行分析。結(jié)果神經(jīng)根型頸椎病的三維平衡正脊手法治療能有效減輕患者臨床癥狀,手法治療后觀察到的影像學(xué)指標(biāo):頸椎生理曲度、頸椎棘突與中線的關(guān)系及椎間孔改變均較治療前有明顯變化,統(tǒng)計(jì)學(xué)分析有意義(P0.05)。結(jié)論1、三維平衡正脊手法治療神經(jīng)根型頸椎病療效肯定。2、三維平衡正脊手法對(duì)神經(jīng)根型頸椎病的治療安全、有效,其機(jī)制可能是調(diào)整了頸椎生物力學(xué)平衡,放松緊張的肌肉,改善局部血液循環(huán),改善小關(guān)節(jié)的紊亂,改善神經(jīng)與突出椎間盤的相對(duì)位置關(guān)系,相對(duì)擴(kuò)大椎間孔與椎間隙的距離,松解神經(jīng)根的黏連,最終改善了患者的臨床癥狀。3、頸椎螺旋CT掃描并三維立體重建較清楚的顯示了手法治療前后神經(jīng)根型頸椎病的影像學(xué)變化:生理曲度的改善、棘突位置的恢復(fù)、椎間孔的擴(kuò)大等等。頸椎螺旋CT掃描并三維立體重建在評(píng)價(jià)神經(jīng)根型頸椎病的治療中具有廣泛應(yīng)用前景。
[Abstract]:Objective to evaluate the effect of three-dimensional balanced spinal manipulation on cervical spondylosis of nerve root type by using multi-slice spiral CT scanning technique, and to analyze the changes of C _ 4 C _ 7 segment mechanical model before and after treatment of "three dimensional fixed-point balanced straight ridge" in patients with cervical spondylopathy of nerve root type. Methods Thirty patients with cervical spondylopathy of nerve root type were examined by spiral CT before and after treatment. Three dimensional reconstruction of cervical vertebrae model. After a course of treatment, the changes of spiral CT were analyzed, including the physiological curvature of cervical spine, the relationship between cervical spine spinous process and midline, the changes of intervertebral foramen and cervical intervertebral disc. Results Three-dimensional balanced ridge manipulation treatment of cervical spondylopathy of nerve root type can effectively alleviate the clinical symptoms of the patients. The imaging index observed after manipulation treatment is the physiological curvature of cervical spine. The relationship between spinous process and midline of cervical spine and the change of intervertebral foramen were significantly different from those before treatment. Conclusion 1. Three-dimensional balanced positive ridge manipulation is effective and safe in the treatment of radicular cervical spondylosis, and its mechanism may be to adjust the biomechanical balance of cervical vertebra. Relax tense muscles, improve local blood circulation, improve the disorder of facet joints, improve the relative position of nerve and herniated intervertebral disc, expand the distance between intervertebral foramen and intervertebral space, release the adhesion of nerve root, Finally, the clinical symptoms of the patients were improved. The spiral CT scan and three-dimensional reconstruction of the cervical spine clearly showed the imaging changes of cervical spondylosis of nerve root type before and after manual treatment: the improvement of physiological curvature, the restoration of the position of spinous process, The enlargement of intervertebral foramen and so on. The spiral CT scan and three-dimensional reconstruction of cervical vertebrae have a wide application prospect in evaluating the treatment of cervical spondylopathy of nerve root type.
