【摘要】：目的探討重型顱腦損傷患者出現(xiàn)嚴(yán)重凝血功能異常的危險因素。方法將96例重型顱腦損傷患者的性別、年齡、受傷至入院時間、入院時格拉斯哥昏迷評分、是否多器官損傷、是否低血壓、是否低血氧、是否手術(shù)治療及CT復(fù)查是否出現(xiàn)顱內(nèi)血腫增大進(jìn)行記錄。同時對入院時凝血酶原時間、活化部分凝血活酶時間、纖維蛋白原及血小板情況進(jìn)行記錄,在住院過程中對患者的凝血功能情況進(jìn)行追蹤。如患者出現(xiàn)嚴(yán)重凝血功能異常視為陽性事件。采用二分類逐步logistic回歸分析數(shù)據(jù)。結(jié)果患者的年齡、格拉斯哥昏迷評分及多器官損傷是患者發(fā)生凝血功能嚴(yán)重異常的獨立影響因素。結(jié)論重型顱腦損傷患者如果存在高齡、格拉斯哥昏迷評分低及多器官損傷,嚴(yán)重凝血功能異常的發(fā)生率較高。
[Abstract]:Objective to investigate the risk factors of severe coagulation dysfunction in patients with severe craniocerebral injury. Methods 96 patients with severe craniocerebral injury were divided into three groups: sex, age, time from injury to admission, Glasgow coma score on admission, and multiple organ injury. At the same time, the time of prothrombin, activated partial thromboplastin, fibrinogen and platelet were recorded. In the course of hospitalization, the coagulation function of the patients was tracked. If the patients with severe abnormal coagulation function were regarded as positive events, the data of stepwise logistic regression analysis were used to analyze the age of the patients. Glasgow coma score and multiple organ injury were independent influencing factors of severe abnormal coagulation function in patients with severe craniocerebral injury. Conclusion if the patients with severe craniocerebral injury have old age, Glasgow coma score is low and multiple organ injury. The incidence of severe coagulation dysfunction was higher.
