【摘要】：[目的]探討髕下脂肪墊(infrapatellar fat pad,IPFP)體積與膝關(guān)節(jié)骨關(guān)節(jié)炎(Keen Osteoarthritis,KOA)的關(guān)系。[方法]對體檢職工進行橫斷面研究,排除膝關(guān)節(jié)外傷、感染、免疫性疾病后,共納入114位受試者(114膝)行膝關(guān)節(jié)MRI掃描。利用mimics16.0處理T1加權(quán)圖像測定IPFP體積;利用質(zhì)子密度加權(quán)圖像對膝關(guān)節(jié)軟骨缺損、骨贅評分;利用WOMAC量表評估膝關(guān)節(jié)疼痛。分析IPFP體積與軟骨缺損、骨贅、疼痛的相關(guān)性;并進一步把受試者分為OA組和對照組,比較組間IPFP體積差異性。[結(jié)果]多因素分析顯示IPFP體積與股骨外側(cè)、脛骨內(nèi)側(cè)、脛骨外側(cè)、全膝關(guān)節(jié)骨贅及睡眠、平地行走時膝關(guān)節(jié)疼痛呈顯著負相關(guān);OA組IPFP體積小于非OA組,但差異無統(tǒng)計學(xué)意義。[結(jié)論]IPFP體積減少與膝關(guān)節(jié)退變有相關(guān)性,但KOA患者IPFP體積較健康人減少并不顯著,提示力學(xué)作用可能不是IPFP影響KOA的主要途徑。
[Abstract]:[objective] to investigate the relationship between the volume of infrapatellar fat fat and knee osteoarthritis (KOAA). The volume of IPFP was measured by mimics16.0 on T1 weighted images, and the cartilage defects and osteophyte scores were evaluated by proton density weighted images. The WOMAC scale was used to evaluate knee joint pain. The correlation between IPFP volume and cartilage defect, osteophyte and pain was analyzed, and the subjects were further divided into OA group and control group. [results] Multivariate analysis showed that the volume of IPFP was negatively correlated with the lateral femur, the medial tibia, the lateral tibia, the total knee osteophyte and sleep, and the knee joint pain when walking flat. The volume of IPFP in OA group was lower than that in non-OA group. But the difference was not statistically significant. [conclusion] there is a correlation between the volume reduction of IPFP and degeneration of knee joint, but the decrease of IPFP volume in KOA patients is not significant compared with that in healthy people, suggesting that the mechanical effect may not be the main way of IPFP affecting KOA.
