氨甲環(huán)酸減少雙側(cè)全膝關(guān)節(jié)置換術(shù)失血量的有效性及安全性研究

【摘要】:目的:探討氨甲環(huán)酸對(duì)雙側(cè)全膝關(guān)節(jié)置換術(shù)患者圍術(shù)期出血量、血紅蛋白變化的影響及其安全性的研究。方法:選取本院69例需進(jìn)行雙側(cè)全膝關(guān)節(jié)置換術(shù)的患者,隨機(jī)分為2組,實(shí)驗(yàn)組給予氨甲環(huán)酸1g溶于100ml 5%葡萄糖溶液中靜脈滴注,對(duì)照組給予5%葡萄糖溶液100ml靜脈滴注。比較兩組的總失血量、術(shù)中失血量、隱性失血量、術(shù)后引流量、輸血率、血紅蛋白、D-二聚體、凝血酶原時(shí)間和活化的部分凝血酶時(shí)間等指標(biāo)的動(dòng)態(tài)變化情況,并研究兩組血栓事件的發(fā)生率。結(jié)果:實(shí)驗(yàn)組的失血量明顯低于對(duì)照組(t=2.194~2.908,P0.05),輸血率也明顯低于對(duì)照組(χ2=5.219,P=0.022)。比較術(shù)后第3天和第5天的血紅蛋白量,實(shí)驗(yàn)組明顯高于對(duì)照組(t=-3.481~2.319,P0.05)。術(shù)前術(shù)后兩組觀察者的凝血酶原時(shí)間、活化的部分凝血酶時(shí)間無明顯差異(P0.05)。結(jié)論:氨甲環(huán)酸能顯著減少雙側(cè)全膝關(guān)節(jié)置換術(shù)患者的圍術(shù)期失血量,降低對(duì)輸血的需求,減小血紅蛋白的動(dòng)態(tài)變化,同時(shí)不增加血栓形成的風(fēng)險(xiǎn)。
[Abstract]:Objective: to study the effect of carbamoic acid on perioperative bleeding, hemoglobin change and safety of bilateral total knee arthroplasty. Methods: 69 patients with bilateral total knee arthroplasty in our hospital were selected. Two groups were randomly divided into two groups: the experimental group was given 1 g of methacylic acid dissolved in 100ml 5% glucose solution, and the control group was given 5% glucose solution 100ml intravenous drip. The total blood loss, intraoperative blood loss, recessive blood loss and postoperative drainage were compared between the two groups. The dynamic changes of blood transfusion rate, hemoglobin D-dimer, prothrombin time and activated partial thrombin time, Results: the blood loss in the experimental group was significantly lower than that in the control group, and the blood transfusion rate was significantly lower than that in the control group. There was no significant difference in prothrombin time and activated partial thrombin time between the two groups before and after operation. Conclusion: the perioperative blood loss of bilateral total knee arthroplasty patients can be significantly reduced by methacylic acid. Reduce the need for blood transfusions, reduce the dynamic changes in hemoglobin, without increasing the risk of thrombosis.
