【摘要】：[目的]探討經(jīng)皮椎弓根螺釘內(nèi)固定術(shù)治療胸腰椎骨折的療效。[方法]將本院2010年1月~2014年12月收治的84例胸腰椎骨折患者隨機(jī)分成2組,每組42例。觀察組采用經(jīng)皮椎弓根螺釘內(nèi)固定術(shù),對(duì)照組采用開放椎弓根螺釘內(nèi)固定術(shù)。對(duì)兩組圍術(shù)期參數(shù)、手術(shù)前后椎體前緣高度和傷椎Cobb角、Frankel脊髓功能進(jìn)行觀察。[結(jié)果]觀察組患者手術(shù)時(shí)間、出血量、切口長(zhǎng)度、術(shù)后1 d疼痛評(píng)分均顯著低于對(duì)照組(P0.05);兩組術(shù)后1年椎體前緣高度百分比顯著高于術(shù)前水平(P0.05),兩組術(shù)后1年Cobb角顯著低于術(shù)前水平(P0.05),兩組間比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);觀察組Frankel脊髓功能評(píng)定結(jié)果顯著優(yōu)于對(duì)照組(P0.05)。[結(jié)論]經(jīng)皮椎弓根螺釘內(nèi)固定術(shù)治療胸腰椎骨折效果滿意,相比開放手術(shù),該術(shù)式具有手術(shù)時(shí)間、出血量、切口長(zhǎng)度、術(shù)后1 d疼痛輕、術(shù)后功能恢復(fù)快的優(yōu)點(diǎn)。
[Abstract]:[objective] to evaluate the efficacy of percutaneous pedicle screw fixation in the treatment of thoracolumbar fractures. [methods] Eighty-four patients with thoracolumbar fractures from January 2010 to December 2014 were randomly divided into two groups. 42 cases in each group were treated with percutaneous pedicle screw fixation in the observation group and open pedicle screw fixation in the control group. The anterior height of vertebral body and the function of Frankel spinal cord in Cobb angle of injured vertebra were observed before and after operation. 1 day after operation, the pain score was significantly lower than that in the control group (P 0.05), the percentage of anterior height of vertebral body in both groups was significantly higher than that before operation (P 0.05), and the Cobb angle was significantly lower in both groups at 1 year after operation than that at preoperative level (P 0.05). There was no significant difference between the two groups. The evaluation results of Frankel spinal cord function in the observation group were significantly better than those in the control group. [conclusion] the effect of percutaneous pedicle screw internal fixation for thoracolumbar fractures is satisfactory. Compared with open operation, it has the advantages of operation time, blood loss, incision length, light pain 1 day after operation and quick recovery of postoperative function.
