【摘要】：股骨頭壞死,又稱股骨頭缺血性壞死,是由不同病因造成的一種病理狀態(tài),包括濫用激素、酗酒、創(chuàng)傷、血管損傷及一些特發(fā)性疾病。股骨頭壞死造成了股骨頭軟骨下骨血供的減少,從而導(dǎo)致了成骨細(xì)胞的死亡及關(guān)節(jié)面的塌陷。股骨頭一旦發(fā)生塌陷,大部分患者將不可避免地進(jìn)行髖關(guān)節(jié)置換手術(shù)。但髖關(guān)節(jié)置換手術(shù)并不是每個(gè)患者的最佳選擇,除了關(guān)節(jié)假體本身的因素外,部分患者的年齡與疾病特點(diǎn)可能會(huì)面臨人工關(guān)節(jié)多次翻修的風(fēng)險(xiǎn),因而一些學(xué)者更提倡股骨頭壞死的保髖治療。目前,股骨頭壞死的保髖手術(shù)治療主要包括截骨術(shù)、髓芯減壓術(shù)、植骨術(shù)、鉭棒置入術(shù)和干細(xì)胞治療等,文章從不同機(jī)制上對(duì)近年來常見的一些保髖手術(shù)進(jìn)行綜述。
[Abstract]:Avascular necrosis of the femoral head, also known as avascular necrosis of the femoral head, is a pathological condition caused by different causes, including hormone abuse, alcoholism, trauma, Vascular injury and some idiopathic diseases. Necrosis of the femoral head causes a decrease in the blood supply of the femoral head subchondral bone, leading to the death of osteoblasts and the collapse of the articular surface. Once the femoral head collapses, Most patients will inevitably undergo hip replacement. But hip replacement is not the best choice for every patient, except for the prosthesis itself. The age and disease characteristics of some patients may face the risk of multiple revision of artificial joints. Therefore, some scholars advocate hip preservation therapy for osteonecrosis of the femoral head. At present, hip preservation surgery for osteonecrosis of the femoral head mainly includes osteotomy. Core decompression, bone grafting, tantalum rod implantation and stem cell therapy are reviewed in this paper.
