【摘要】：目的總結(jié)顱耳溝小切口微創(chuàng)手術(shù)治療先天性隱耳畸形的療效。方法 2011年1月-2015年5月收治先天性隱耳畸形8例,其中男6例,女2例;年齡5~13歲,平均10歲。單側(cè)3例,雙側(cè)5例。13只隱耳均可用外力牽拉復(fù)位至正常大小形態(tài)及顱耳溝。于顱耳溝內(nèi)設(shè)計長約3 cm弧形切口,并充分松解耳廓軟骨與顱側(cè)面、耳廓上部軟骨間及軟骨和皮膚之間的纖維粘連,游離顱側(cè)面皮膚向顱耳溝推進修復(fù)創(chuàng)面缺損,術(shù)后紗布卷固定支撐耳廓3周。結(jié)果術(shù)后8例患者13只隱耳畸形均矯正滿意;術(shù)后3周拆除支撐紗布卷后耳廓無回縮,形態(tài)正常。8例患兒均獲隨訪,隨訪時間1~3年,平均1年6個月。耳廓外形無明顯回縮。1例術(shù)后1周耳后皮膚部分壞死,為紗布卷偏大致皮膚長期受壓缺血所致,去除紗布卷后換藥愈合。結(jié)論顱耳溝小切口微創(chuàng)手術(shù)是治療先天性隱耳畸形有效而簡便的手術(shù)方法 。
[Abstract]:Objective to summarize the curative effect of minimally invasive surgical treatment of congenital saphenous ear malformation with small incision in cranial ear sulcus. Methods from January 2011 to May 2015, 8 cases of congenital saphenous ear malformation were treated, including 6 males and 2 females, with a mean age of 10 years. In 5 cases, 13 ears could be reduced to normal size and cranial sulcus by external force. A 3cm arc incision was designed in the sulcus of the cranial ear, and the auricle cartilage and the lateral side of the skull were fully released. The fibrous adhesion between the upper auricle and between the cartilage and the skin, the free lateral skin of the skull pushed to the cranial ear groove to repair the defect of the wound, and the gauze coil fixed the auricle to support the auricle for 3 weeks. Results the correction of 13 saphenous ear deformities in 8 patients after operation was satisfactory. After 3 weeks removal of the supporting gauze, there was no retraction of the auricle, and 8 cases with normal shape were followed up for 1 ~ 3 years, with an average of 1 year and 6 months. There was no obvious retraction of the auricle in 1 week after operation. In order to induce long-term compression and ischemia of the skin of gauze roll, dressing change and healing after removal of gauze coil. Conclusion minimally invasive cranioauricular incision is an effective and simple method for the treatment of congenital saphenous ear malformation.
