【摘要】：目的探討應(yīng)用活血通脈片對于治療全髖關(guān)節(jié)置換術(shù)后下肢腫脹疼痛的臨床療效。為預(yù)防及治療術(shù)后下肢腫脹、疼痛及下肢深靜脈血栓等并發(fā)癥提供客觀依據(jù)。方法將60例患者按照隨機對照的原則分為實驗組和對照組。對照組THA后由關(guān)節(jié)骨科醫(yī)護(hù)人員進(jìn)行常規(guī)的護(hù)理及康復(fù)訓(xùn)練指導(dǎo);實驗組THA后由同組關(guān)節(jié)骨科醫(yī)護(hù)人員進(jìn)行相同的常規(guī)護(hù)理及康復(fù)訓(xùn)練指導(dǎo),并于術(shù)后第一天開始口服活血通脈片治療項目至術(shù)后一個月。分別于術(shù)前24h,術(shù)后1d、3d、5d、7d測量兩組患者下肢髕上、髕下周徑及靜息、疼痛時VAS評分變化。結(jié)果1.兩組病人測量髕上15cm、髕下10cm的周徑在術(shù)前24h、術(shù)后1d(治療前)比較,無統(tǒng)計學(xué)意義(P0.05),在術(shù)后第3、5、7天的腫脹指標(biāo)進(jìn)行比較,有統(tǒng)計學(xué)差異(P〈0.05),實驗組的腫脹度均低于對照組。2.兩組病人靜息時疼痛VAS評分術(shù)前24h、術(shù)后1d(治療前)比較,無統(tǒng)計學(xué)意義(P0.05),在術(shù)后第3、5、7天兩組VAS評分比較,有統(tǒng)計學(xué)差異(P〈0.05),比較兩組平均等級,實驗組靜息時的疼痛VAS評分低于對照組。結(jié)論綜合應(yīng)用活血通脈片縮短了全髖關(guān)節(jié)置換術(shù)術(shù)后下肢腫痛的持續(xù)時間,有效減輕了下肢腫脹、疼痛程度,可有效預(yù)防DVT的形成。
[Abstract]:Objective to investigate the clinical effect of Huoxue Tongmai tablet on lower limb swelling and pain after total hip replacement. Methods 60 patients were divided into experimental group and control group according to the principle of random control. The control group was guided by routine nursing and rehabilitation training after THA. After THA in the experimental group, the same routine nursing and rehabilitation training were conducted by the same group of joint orthopedic medical staff. From the first day of operation to one month after operation, the upper patellar diameter, patellar diameter and rest of the lower extremities were measured at 24 hours before operation and 1 day after operation for 5 days and 7 days, respectively. Results 1. The patients in the two groups were measured the superior patellar 15 cm, the circumference of 10 cm below the patella was 24 hours before operation, 1 day after the operation (before treatment), there was no significant difference between the two groups (P 0.05). The swelling index was compared on the 3rd and 5th day after operation. The swelling degree of the experimental group was lower than that of the control group (P < 0.05). The VAS score of pain in the two groups was 24 hours before operation and 1 day after operation (before treatment). There was no significant difference in VAS score between the two groups on the 3rd day after operation (P < 0.05), and on the 7th day after operation, there was no significant difference between the two groups. There was significant difference between the two groups (P < 0.05). Compared with the average grade of the two groups, the VAS score of pain in the experimental group at rest was lower than that in the control group. Conclusion the comprehensive application of Huoxue Tongmai tablet shortens the duration of swelling and pain of lower extremity after total hip arthroplasty, and effectively reduces the swelling of lower extremity. The degree of pain can effectively prevent the formation of DVT.
