去金屬偽影技術(shù)在骨科金屬植入物術(shù)后計(jì)算機(jī)斷層掃描圖像中的應(yīng)用

【摘要】:目的探討去金屬偽影(MAC)技術(shù)在骨科金屬植入物術(shù)后CT檢查中的應(yīng)用價(jià)值。方法收集2014年11月—2015年9月間40例在行骨科金屬植入物術(shù)后接受16排CT掃描檢查的患者資料,對(duì)其同一掃描的原始數(shù)據(jù),分別采用設(shè)備自帶的標(biāo)準(zhǔn)濾波反投影重建算法(常規(guī)重建組)和MAC技術(shù)的去金屬偽影重建法(MAC重建組)進(jìn)行圖像后處理。由3名有經(jīng)驗(yàn)的放射科醫(yī)師采用雙盲法對(duì)兩組圖像質(zhì)量進(jìn)行客觀、主觀評(píng)價(jià),客觀評(píng)價(jià)指標(biāo)包括圖像噪聲值(SD)、信噪比(SNR)、對(duì)比噪聲比(CNR),主觀評(píng)價(jià)采用5分賦值法。結(jié)果 MAC重建組CT圖像的SNR、CNR均顯著大于常規(guī)重建組(P值均0.01),金屬植入物相鄰肌肉的噪聲值(SD1)、相同測(cè)量層面皮下脂肪的噪聲值(SD2)均顯著小于常規(guī)重建組(P值均0.01)。MAC重建組的主觀評(píng)價(jià)評(píng)分為(4.37±0.20)分,顯著高于常規(guī)重建組的(2.45±0.15)分(P0.01)。3名放射科醫(yī)師CT圖像質(zhì)量主觀評(píng)價(jià)的一致性良好(Kappa值=0.89)。與常規(guī)重建組多平面重建(MRR)模式的冠狀位CT圖像和容積再現(xiàn)(VR)模式重建圖像相比,MAC重建組圖像的偽影對(duì)周圍組織的干擾明顯減小,能夠清晰地觀察金屬植入物周圍的骨質(zhì)、軟組織情況,圖像噪聲明顯降低。結(jié)論 MAC技術(shù)能顯著消除金屬偽影,提高金屬植入物受檢部位CT圖像的質(zhì)量,且周圍軟組織清晰可見,該技術(shù)對(duì)骨科金屬植入物術(shù)后CT圖像分析具有良好的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the value of metal removal artifact (Marc) technique in CT examination after orthopedic metal implants. Methods 40 patients undergoing 16 row CT scan after orthopedic metal implants from November 2014 to September 2015 were collected. Raw data for the same scan, Standard filter back-projection reconstruction algorithm (conventional reconstruction group) and de-metal artifact reconstruction method (MAC technique) were used for image post-processing respectively. Three experienced radiologists used double-blind method. The quality of the two groups of images is objective, Subjective evaluation, Objective evaluation indexes included image noise value, SNR and SNR ratio. Subjective evaluation was performed by 5 points assignment method. Results the SNR of MAC reconstruction group was significantly higher than that of conventional reconstruction group (P < 0.01), and the adjacent muscles of metal implants were significantly higher than those of conventional reconstruction group (P < 0.01). The noise value of meat and the noise value of subcutaneous fat measured on the same level were significantly lower than those of the conventional reconstruction group (P = 0.01) and the subjective evaluation score of MAC reconstruction group was 4.37 鹵0.20, respectively. It was significantly higher than that of conventional reconstruction group (2.45 鹵0.15). The consistency of subjective evaluation of CT image quality of 3 radiologists was good. The Kappa value was 0.890.It was higher than that of conventional reconstruction group by coronal CT and volumetric reconstruction with VRR. Compared with MAC reconstruction group, the artifacts of the reconstructed group obviously reduced the interference to the surrounding tissues. It can clearly observe the bone and soft tissue around the metal implants, and the image noise is obviously reduced. Conclusion MAC technique can significantly eliminate metal artifacts and improve the quality of CT images of metal implants. And the surrounding soft tissue is clearly visible. This technique has a good clinical value in the analysis of CT images after orthopedic metal implants.
