【摘要】：目的探討CD64指數(shù)在特重度燒傷膿毒癥患者預(yù)后評(píng)估中的價(jià)值。方法選取燒傷膿毒癥住院患者32例,按結(jié)局分為死亡組(18例)和生存組(14例)�；颊呷朐汉蟛捎弥苯用庖邿晒夥y(cè)定CD64指數(shù);采用比濁儀測(cè)定CRP;血液分析儀檢測(cè)外周血白細(xì)胞計(jì)數(shù)(WBC)和中性粒細(xì)胞比率(N)。在抗感染治療過(guò)程中持續(xù)檢測(cè)上述各指標(biāo)。用ROC曲線(xiàn)分析患者年齡和入院第28天CD64指數(shù)對(duì)燒傷膿毒癥患者預(yù)后的判斷效果及其靈敏度和特異度。結(jié)果兩組患者確診膿毒癥時(shí)各項(xiàng)指標(biāo)比較差異均無(wú)統(tǒng)計(jì)學(xué)意義。生存組患者入院后第1、7、14天CD64指數(shù)上升,在第14天達(dá)到最高水平,然后逐漸下降;而死亡組患者CD64指數(shù)從第1天~第35天逐漸上升。在第28天和第35天兩組CD64指數(shù)比較有統(tǒng)計(jì)學(xué)差異(P0.01)。年齡對(duì)燒傷膿毒癥預(yù)后判斷的最佳界值為33.5歲,靈敏度和特異度分別為0.933和0.778。入院第28天CD64指數(shù)對(duì)燒傷膿毒癥患者預(yù)后判斷的最佳界值為0.385,靈敏度和特異度分別為0.933和0.889。結(jié)論 CD64指數(shù)能及時(shí)評(píng)估特重度燒傷膿毒癥患者病情與預(yù)后情況,對(duì)盡早實(shí)施臨床干預(yù)和治療有重要指導(dǎo)意義。
[Abstract]:Objective to evaluate the prognostic value of CD64 index in patients with severe burn sepsis. Methods 32 patients with burn sepsis were selected. The patients were divided into death group (n = 18) and survival group (n = 14) according to the outcome. The CD64 index was measured by direct immunofluorescence assay after admission. CRP was measured by turbidimeter, WBC count and neutrophil ratio by blood analyzer. The above indexes were continuously detected during anti-infection therapy. The age of patients and the number of CD64 fingers on the 28th day of admission were analyzed by ROC curve. Results there was no significant difference in the indexes between the two groups in the diagnosis of sepsis. The CD64 index of the patients in the survival group increased 714 days after admission. Reached the highest level on the 14th day, then gradually decreased; In the death group, the CD64 index increased gradually from day 1 to day 35. There was a significant difference in CD64 index between the two groups on the 28th and the 35th day. The best threshold of age for the prognosis of burn sepsis was 33.5 years old. The sensitivity and specificity were 0.933 and 0.778, respectively. The best value of CD64 index for the prognosis of burn sepsis patients was 0.385 on the 28th day after admission, and the sensitivity and specificity were 0.933 and 0.889 respectively. Conclusion the CD64 index can be used to evaluate the severe burn pus in time. The condition and prognosis of toxic patients, It has important guiding significance for clinical intervention and treatment as soon as possible.
