骨性關節(jié)炎患者膝關節(jié)軟骨損傷的關節(jié)鏡與MRI診斷分級研究

【摘要】:[目的]提出關節(jié)鏡及核磁共振成像(magnetic resonance imaging,MRI)診斷骨性關節(jié)炎(osteoarthrisis,OA)患者膝關節(jié)軟骨損傷的分級法,探討MRI在診斷關節(jié)軟骨損傷中的價值。[方法]回顧性分析行關節(jié)鏡檢查的123例骨性關節(jié)炎患者的膝關節(jié)術前MRI表現(xiàn);按部位將膝關節(jié)軟骨分為6處;根據(jù)形態(tài)和信號改變,結合損傷部位,將膝關節(jié)軟骨損傷在關節(jié)鏡與MRI的表現(xiàn)加以分析并分級;以關節(jié)鏡所見為參考標準,計算MRI診斷各級軟骨損傷的敏感性、特異性、準確性。[結果]在關節(jié)鏡及MRI上,膝關節(jié)軟骨損傷均可分為Ⅰ~Ⅳ級;在738處軟骨中,損傷505處(68.43%),其中髕股關節(jié)最多,外側脛股關節(jié)最少,且損傷以Ⅲ~Ⅳ級為主;關節(jié)鏡與MRI判斷軟骨損傷差異具有統(tǒng)計學意義(χ~2=107.52,P0.001)。MRI診斷軟骨損傷總的敏感性、特異性、準確性分別為70.10%、93.13%、77.37%。[結論]骨性關節(jié)炎患者膝關節(jié)軟骨損傷在關節(jié)鏡及MRI的表現(xiàn)可被分為4級;軟骨損傷以髕股關節(jié)最多;MRI診斷骨性關節(jié)炎軟骨損傷的特異性較高,但敏感性和準確性相對較低。
[Abstract]:[Objective] to study the diagnosis of osteoarthrisis by arthroscopy and magnetic resonance imaging (resonance). To evaluate the value of MRI in the diagnosis of articular cartilage injury. [Methods: the preoperative MRI findings of 123 patients with osteoarthritis underwent arthroscopic examination were retrospectively analyzed. The articular cartilage was divided into 6 parts according to the position. According to the changes of morphology and signal, combined with the injury site, the articular cartilage injuries were analyzed and graded under arthroscopy and MRI. The sensitivity, specificity and accuracy of MRI in the diagnosis of cartilage injury at all levels were calculated according to arthroscopy.
